Purpose: Immune Checkpoints Inhibitors (ICI) have changed the therapeutic landscape of metastatic renal cell carcinoma first-line treatment with complete response (CR) at metastatic sites observed in 10 to 15% of cases. Delayed nephrectomy could be discussed for patients having a clinical benefit from immunotherapy-based treatment. However, it is unclear whether prior immunotherapy exposure adversely influences the complexity of surgery.

The ever-increasing use of immune checkpoint inhibitors (ICIs) has significantly improved cancer management, but at the cost of frequent immunologic side effects. Among them, neurologic immune-related adverse events (nirAEs) are less common but pose a challenge to clinicians due to their severity, heterogeneous nature and nonspecific clinical presentation, making diagnosis complex. The prognosis of these nirAEs, especially those related to the central nervous system (CNS), correlates with their rapid recognition and therapeutic management.

Article Synopsis
  • - Merkel cell carcinoma is a rare and aggressive skin cancer characterized by a high likelihood of spreading and a generally poor prognosis, with treatment options including surgery, radiation, immunotherapy, and chemotherapy.
  • - A case study describes a 65-year-old woman who received local radiation therapy for an advanced Merkel cell carcinoma on her buttock, along with chemotherapy, and experienced a pancreatic metastasis as well.
  • - Remarkably, after treating an isolated pancreatic recurrence with radiation, she achieved complete remission and was still alive 13 years later, suggesting that radiation therapy can be a viable treatment for advanced Merkel cell carcinomas.

JCO GETUG-13 established that switching patients with poor-prognosis nonseminomatous germ-cell tumors with an unfavorable marker decline to intensified chemotherapy resulted in improved outcomes. Here, we report the GETUG-13 long-term efficacy and toxicity. Two hundred and sixty-three patients with International Germ Cell Cancer Consensus Group poor prognosis received one cycle of bleomycin, etoposide, and cisplatin (BEP): 51 with a favorable tumor marker decline continued with three cycles of BEP (Fav-BEP) and 203 with an unfavorable decline were randomly treated with three BEP (Unfav-BEP) cycles or a dose-dense regimen (Unfav-dose-dense; two cycles of paclitaxel-BEP-oxaliplatin + two cycles of cisplatin, ifosfamide, and bleomycin).

Introduction: Immune checkpoint inhibitors are standard of care in metastatic renal cell carcinoma but their activity and safety in elderly patients is insufficiently explored. We evaluated outcomes of elderly patients with mRCC treated with nivolumab in the GETUG-AFU 26 NIVOREN phase 2 trial (NCT03013335) and conducted exploratory circulating biomarker analyses.

Methods: Patients with mRCC were treated with nivolumab after at least one antiangiogenic therapy.

Article Synopsis
  • There is currently no reliable biomarker for predicting the effectiveness of EGFR inhibitors in head and neck squamous cell carcinoma (HNSCC), leading researchers to investigate afatinib, a promising treatment.
  • In a trial with 61 patients, afatinib showed a high metabolic response, with responders demonstrating specific biomarker expressions (higher pERK1/2 and lower pHER4 and pRB1 levels).
  • Further analysis indicated that responders had more tumor-infiltrating B cells, while non-responders had an overrepresentation of NF-kappa B signaling in their gene expression, suggesting potential new biomarkers for afatinib efficacy that require more research.*

Background: Papillary renal cell carcinoma (pRCC) is the most common non-clear cell RCC, and associated with poor outcomes in the metastatic setting. In this study, we aimed to comprehensively evaluate the immune tumor microenvironment (TME), largely unknown, of patients with metastatic pRCC and identify potential therapeutic targets.

Methods: We performed quantitative gene expression analysis of TME using Microenvironment Cell Populations-counter (MCP-counter) methodology, on two independent cohorts of localized pRCC (n=271 and n=98).

Article Synopsis
  • Glandular metastases (GMs) are uncommon in metastatic clear cell renal cell carcinoma (mccRCC), but patients with GMs treated with antiangiogenic therapy show better survival outcomes.
  • The GETUG-AFU-26 NIVOREN trial analyzed the effects of nivolumab on mccRCC patients, categorizing them based on the presence of GMs to compare survival outcomes.
  • Among 720 patients, those with adrenal GMs had poorer overall survival and response rates, while those with pancreatic GMs had significantly better survival rates compared to nonpancreatic metastasis.

Background: Despite metastatic renal cell carcinoma (mRCC) expanded treatment options, disease progression ultimately occurs for most patients. Rechallenge may be a compelling strategy in a refractory setting. Cabozantinib is the standard of care in first and later lines of therapy, but its activity in rechallenge is unknown.

Purpose: Patients with locally advanced or metastatic urothelial cancer (la/mUC) who are ineligible for cisplatin-based therapy have limited first-line (1L) treatment options and significant need for improved therapies. Enfortumab vedotin (EV) and pembrolizumab (Pembro) individually have shown a survival benefit in urothelial cancer in second-line + la/mUC settings. Here, we present data from the pivotal trial of EV plus Pembro (EV + Pembro) in the 1L setting.

Background: With more than 15,000 new cases /year in France and 2,000 deaths, cutaneous melanoma represents approximately 4% of incidental cancers and 1.2% of cancer related deaths. In locally advanced (stage III) or resectable metastatic (stage IV) melanomas, medical adjuvant treatment is proposed and recent advances had shown the benefit of anti-PD1/PDL1 and anti-CTLA4 immunotherapy as well as anti-BRAF and anti-MEK targeted therapy in BRAF V600 mutated tumors.

Introduction: Bone metastases (BM) in renal cell carcinoma (RCC) are associated with a poor prognosis based on retrospective studies evaluating antiangiogenic agents. Few data are available regarding immune checkpoint inhibitors (ICI) in patients with bone metastatic RCC. NIVOREN is a multicentre prospective study in which patients were treated with nivolumab after the failure of antiangiogenic agents.

Article Synopsis
  • Proton pump inhibitors (PPIs) might affect the gut microbiome and could influence the effectiveness of immune checkpoint inhibitors (ICIs) in metastatic renal cell carcinoma (mRCC), but this has not been fully studied.
  • A retrospective analysis of 707 patients from a phase II study found that 27.7% were PPI users, mostly males with good performance status, but their PPI use did not show a significant impact on progression-free survival (PFS) or overall survival (OS).
  • However, PPI users experienced a higher rate of severe nivolumab-related side effects, indicating that while PPI may not affect treatment efficacy, it could impact safety, suggesting the need for further research.

Background: MBD4 mutations have been reported in uveal melanomas, acute myeloid leukemias, colorectal adenocarcinomas, gliomas, and spiradenocarcinomas and cause a hypermutated phenotype. Although metastatic uveal melanomas (mUM) are usually resistant to immune checkpoint inhibitors (ICI), the first reported MBD4-mutated (MBD4m) patient responded to ICI, suggesting that MBD4 mutation may predict response to ICI.

Methods: Retrospective cohort of mUM patients treated with ICI.

Background: Tyrosine kinase inhibitors (TKIs) remain a cornerstone of metastatic kidney cancer (mRCC). Adverse events (AEs) may lead to dose downregulation, and optimal management of AEs is needed to maintain an efficient dose intensity (DI). The aim of our study was to evaluate the impact of an app-based and nurse-led supportive-care program on DI in mRCC patients.

Article Synopsis
  • Cisplatin-based combination chemotherapy is the standard treatment for muscle-invasive bladder cancer before surgery, but the best chemotherapy options are still unclear.
  • In the VESPER trial, patients were given either gemcitabine and cisplatin (GC) or a more intense regimen called dd-MVAC, and their kidney function was monitored during treatment.
  • Results showed that at least 4 cycles of cisplatin are necessary for effective treatment, and while extending beyond 4 cycles does not worsen kidney function, it also does not provide significant additional benefits in local cancer control.

Primary tumour response may impact therapeutic strategies in metastatic renal cell carcinoma (mRCC) but remains unknown in the era of immune checkpoint inhibitors. We aimed to describe the response of the primary tumour in patients who did not undergo upfront cytoreductive nephrectomy (uCN) and were treated with nivolumab in the GETUG-AFU-26 NIVOREN phase 2 trial. Primary tumour response was prospectively assessed, as well as the overall response rate (ORR), progression-free survival (PFS), and overall survival (OS).
