Most studies on surgical site infections (SSIs) use the US Centers for Disease Control and Prevention (CDC) guidelines, which excludes inflammation or cellulitis without fever. The aim of this study was to evaluate antibiotic prescription trends in the post-operative period among patients undergoing breast operation. We explore the outcomes of patients receiving antibiotic agents outside of the CDC-defined SSI.

Article Synopsis
  • This study examines the link between food insecurity and weight loss outcomes in patients who underwent Roux-en-Y gastric bypass (RYGB) or sleeve gastrectomy (SG), given previous findings that food insecurity can contribute to obesity and hinder weight loss efforts.
  • Analysis involved over 5,000 patients from the Cleveland Clinic, categorizing them based on their food security status using a modified Retail Food Environmental Index (mRFEI), and tracking their weight changes post-surgery for around 2.3 years.
  • Results revealed that food security status did not significantly affect weight loss, weight loss peaks, or nutritional deficiencies after surgery, suggesting that metabolic surgery is effective regardless of a patient's food environment.*

Breast cancer survival has increased significantly over the last few decades due to more effective strategies for prevention and risk modification, advancements in imaging detection, screening, and multimodal treatment algorithms. However, many have observed disparities in benefits derived from such improvements across populations and demographic groups. This review summarizes published works that contextualize modern disparities in breast cancer prevention, diagnosis, and treatment and presents potential strategies for reducing disparities.

Purpose/objective(s): Accelerated partial breast irradiation (PBI) delivered in 5 fractions with intensity modulated radiation therapy (IMRT) has been shown to have comparable clinical outcomes to whole breast irradiation with reduced toxicity profiles. In contrast, intraoperative radiation therapy (IORT) offers patients the potential to complete adjuvant radiation therapy in a single treatment. While early data were promising, concerns exist regarding long-term rates of local recurrence after IORT.

Background: Increasingly, data have supported the use of partial-breast irradiation (PBI) for low-risk patients after breast-conserving surgery, with techniques allowing for completion of treatment in 1-3 weeks. Intraoperative radiation therapy (IORT) is an alternative to PBI. Our institution had used low-energy photon IORT (TARGIT) for more than a decade.

Objective: Discuss the evolution of mentorship models in surgical training and how educating the surgical trainees with the concepts from "manage up" theory can empower them to maximize the benefits afforded by mentoring relationships.

Methods: "Manage up" theory is derived from the business world where the subordinate takes ownership of the mentoring relationship with their superior by assessing the strengths and weaknesses of both parties and applying that information in managing a productive relationship.

Discussion: Surgery residency programs implement a variety of structured and unstructured mentorship programs to promote mentoring relationships and to encourage professional development.

We report a new mechanism of androgen receptor (AR) mRNA regulation and cytoprotection in response to AR pathway inhibition (ARPI) stress in prostate cancer (PCA). AR mRNA translation is coordinately regulated by RNA binding proteins, YTHDF3 and G3BP1. Under ambient conditions m6A-modified AR mRNA is bound by YTHDF3 and translationally stimulated, while m6A-unmodified AR mRNA is bound by G3BP1 and translationally repressed.

Understanding the electronic structure and dynamics of semiconducting nanomaterials at the atomic level is crucial for the realization and optimization of devices in solar energy, catalysis, and optoelectronic applications. We report here on the use of ultrafast X-ray linear dichroism spectroscopy to monitor the carrier dynamics in epitaxial ZnO nanorods after band gap photoexcitation. By rigorously subtracting out thermal contributions and conducting calculations, we reveal an overall depletion of absorption cross sections in the transient X-ray spectra caused by photogenerated charge carriers screening the core-hole potential of the X-ray absorbing atom.

As administrators evaluate potential approaches to improve cost, quality, and throughput efficiencies in the emergency department (ED), "front-end" operations become an important area of focus. Interventions such as immediate bedding, bedside registration, advanced triage (triage-based care) protocols, physician/practitioner at triage, dedicated "fast track" service line, tracking systems and whiteboards, wireless communication devices, kiosk self check-in, and personal health record technology ("smart cards") have been offered as potential solutions to streamline the front-end processing of ED patients, which becomes crucial during periods of full capacity, crowding, and surges. Although each of these operational improvement strategies has been described in the lay literature, various reports exist in the academic literature about their effect on front-end operations.

The term 'whiplash' was initially used to describe injuries to the neck caused by the head being forced backwards during a rear-end collision in cars without head restraints. The addition of head restraints in the 1970s was expected to solve this problem by preventing excessive extension of the neck but experience suggests the problem still exists. This paper reviews available experimental studies of whiplash and uses the data to construct a finite element model which is capable of dynamically simulating whiplash collisions and predicting the forces in all the relevant neck ligaments.

Although providing an historical overview of the development and clinical applications of conduit-mounted prosthetic heart valves, with elementary explanations of operative techniques, this review concentrates on the impact of design and surgical considerations on their in vivo performance. Areas showing potential for design improvement of mechanical prostheses are highlighted, both in terms of haemodynamic efficiency and haemocompatibility. Comparisons are drawn throughout the paper with conventional valve prostheses, and the key features where they differ significantly are pointed out.

This review concerns the issues affecting the in vitro evaluation of conduit-mounted prosthetic heart valves at the design development stage, and the question of standardisation of testing at the quality assurance stage. Particular attention is given to areas of conduit valve development and research which have been neglected, ambiguously covered or left to the discretion of the researcher in the current standard for conventional prosthetic valves, ISO 5840.

Preliminary details are given of a novel method of detecting breast lumps. The technique relies on the fact that such a lump will cause a perturbation in the pressure distribution when the breast is loaded against a flat plate. The pressure distribution is monitored by using the principle of frustrated total internal reflection to generate a brightness distribution which can then be presented as a contour map in synthesized colour.
