Article Synopsis
  • Mpox virus (MPXV) triggered a multi-country outbreak in 2022, leading to the use of the modified vaccinia Ankara (MVA) vaccine for prevention, although its effectiveness has not been fully understood.
  • Researchers conducted assays to measure neutralizing antibodies (NAbs) in individuals infected with MPXV or vaccinated with MVA, finding varying levels of antibodies in both groups.
  • The study revealed that MPXV is less sensitive to neutralization, yet combining it with complement improves detection of antibody responses, underscoring the role of historic smallpox vaccinations in enhancing immune response against MPXV.

Introduction: Leprosy reactions (LRs) are inflammatory responses observed in 30%-50% of people with leprosy. First-line treatment is glucocorticoids (GCs), often administered at high doses with prolonged courses, resulting in high morbi-mortality. Methotrexate (MTX) is an immunomodulating agent used to treat inflammatory diseases and has an excellent safety profile and worldwide availability.

  • Strongyloidiasis is a common and often unnoticed parasitic infection that can lead to serious complications, especially under immunosuppressive conditions like corticosteroid use.
  • The study reports seven cases of larva currens, a rare skin sign associated with strongyloidiasis, observed in Paris from 1990 to 2020, highlighting its clinical features and connections to other symptoms.
  • All cases were successfully treated with ivermectin, emphasizing the importance of early diagnosis and prevention of severe manifestations, particularly in individuals from endemic areas before starting immunosuppressive therapy.

Article Synopsis
  • Monkeypox virus (MPXV) is currently spreading among men who have sex with men, particularly through close contact during sexual activities, prompting research on its distribution in the human body.
  • A study at Pitié-Salpêtrière Hospital in Paris analyzed samples from 50 men with confirmed MPXV infections across multiple anatomical sites, revealing that skin, anus, and throat had higher rates of positive samples and viral loads compared to blood, urine, and semen.
  • Over a two-week period, the proportion of positive samples from these sites significantly decreased, indicating a reduction in viral presence as the infection progressed.

Article Synopsis
  • A quinquagenarian patient with AIDS and fungal endocarditis developed a nosocomial thrombophlebitis in the right jugular vein, which was treated with a combination of antibiotics and therapeutic anticoagulation, leading to a recovery without recurrence after 12 months.
  • The case highlights the challenges of treating septic thrombophlebitis, particularly due to the lack of standardized treatment protocols and concerns about multidrug-resistant bacteria.
  • The findings suggest that effective management should involve removing any medical devices, using appropriate antibiotic therapy for at least six weeks, and administering anticoagulation for a similar duration to reduce bacterial load and minimize resistance.

Article Synopsis
  • - A healthy man developed spondylodiscitis, an infection of the spine, one year after being exposed to a pathogen.
  • - He was successfully treated with a 6-week course of amoxicillin, followed by ciprofloxacin, and did not require surgery.
  • - This case highlights that infections like spondylodiscitis can lead to serious bone and joint issues with delayed symptoms after exposure to the causative bacteria.

Objectives: We report the characteristics of Mycoplasmagenitalium (MG) infection in patients from a STI center in Paris. We evaluated outcomes after treatment.

Methods: We included all patients tested for MG, Chlamydiatrachomatis (CT) and Neisseria gonorrhoeae (NG) infection in our center from January 2017 to December 2018, using multiplex PCR on urine specimen, vaginal or rectal swabs.

Environmental factors were reported to increase the risk of human herpesvirus 8 (HHV-8) transmission. In a population of men who have sex with men (MSM), we found evidence that chemsex was associated with human herpesvirus 8 seropositivity in vivo and that poppers induced HHV-8 virion production in vitro. Our finding may explain the higher HHV-8 transmission in MSM.

We present a case of persistent Mycoplasma genitalium urethritis with documented macrolide and fluoroquinolone resistance, and we describe the A2062T mutation in the 23S rRNA gene, possibly associated with pristinamycin resistance. After several treatment failures and loss of the A2062T mutation, M. genitalium urethritis was finally cured by a sequential antibiotic treatment including minocycline.

Article Synopsis
  • The study aimed to identify the most effective combination of biomarkers for diagnosing infection and sepsis in emergency room patients.
  • Researchers evaluated 30 different biomarkers in a cohort of 291 patients suspected of having infections and used advanced statistical methods to determine the optimal combinations.
  • The findings revealed that specific biomarker combinations could accurately distinguish between bacterial and viral infections, achieving high predictive accuracy with area under the curve (AUC) values of 0.94 and 0.98, respectively.

Background: Data from nonrandomized studies have suggested that hydroxychloroquine could be an effective therapeutic agent against coronavirus disease 2019 (COVID-19).

Methods: We conducted an observational, retrospective cohort study involving hospitalized adult patients with confirmed, mild to severe COVID-19 in a French university hospital. Patients who received hydroxychloroquine (200 mg 3 times daily dosage for 10 days) on a compassionate basis in addition to standard of care (SOC) were compared with patients without contraindications to hydroxychloroquine who received SOC alone.

We report an autochthonous case of oral dirofilariasis in a 46-year-old female patient exposed in South-Eastern France. The patient first presented eyelid creeping dermatitis of one-week duration, then a sub-mucosal nodule appeared in the cheek. The entire nodule was removed surgically.

Background: The incidence of sexually transmitted infections (STI) is increasing in Western countries whilst travel plays a major role in STI dissemination worldwide. However, there is no study distinguishing HIV-positive and HIV-negative travellers.

Methods: We retrospectively evaluated the epidemiological, clinical and biological characteristics of the patients diagnosed with a travel-related STI between 2008 and 2016.

Background: There is no precise idea whether patients with chronic symptoms attributed to Lyme borreliosis (LB) have LB or another disease.

Methods: We evaluated patients consulting for a presumed LB with a holistic approach including presumptive treatment. We included symptomatic patients consulting for presumed LB.
