Elevated intracranial pressure (ICP) is defined as a cerebrospinal fluid (CSF) opening pressure (OP) greater than 25 cmH2O. When a diagnostic lumbar puncture is performed it is useful to estimate also intracranial pressure. We describe the case of an 80-year-old man who came to our observation with pain in his left side and iliac region. Ultrasonography showed an incidentaloma with a largest diameter of 10 cm in the left suprarenal fossa. It extended downward and anteriorly from the apex of the left kidney; the picture was suggestive of a saccate hematoma.
