Article Synopsis
  • The study explored the effectiveness of emergency coronary angiography (CAG) and percutaneous coronary intervention (PCI) in improving survival and neurological outcomes for patients who experienced out-of-hospital cardiac arrest (OHCA) without STEMI.
  • Researchers conducted a randomized trial with 69 OHCA survivors, assigning them to either immediate CAG or deferred CAG, focusing on in-hospital survival and major adverse cardiac events as primary endpoints.
  • Results showed no significant difference in survival rates between the immediate and delayed CAG groups, concluding that immediate intervention did not offer benefits in terms of survival without neurological impairment.

Article Synopsis
  • Dexmedetomidine, a sedative that allows for cooperative sedation, was studied in patients in cardiac intensive care units (CICU) over a year, involving 410 patients with an average age of 67.4 years.
  • Before treatment, 60.2% of patients exhibited delirium, and during dexmedetomidine use, 48 patients developed delirium, with 43.4% of the patients using it while weaning off mechanical ventilation.
  • The study found that adverse effects like hypotension and bradycardia occurred in a small percentage of patients, with risk linked to the use of inotropes and fewer adjuvant sedatives.

Background: Coronary artery disease (CAD) is a major cause of out-of-hospital cardiac arrest (OHCA). The role of emergency coronary angiography (CAG) and percutaneous coronary intervention (PCI) following cardiac arrest in patients without ST-segment elevation myocardial infarction (STEMI) remains unclear.

Aims: We aim to assess whether emergency CAG and PCI, when indicated, will improve survival with good neurological outcome in post-OHCA patients without STEMI who remain comatose.

Introduction and objectives. Troponin T (TnT) is a very specific marker of myocardial damage. Our objective was to describe TnT behavior after dobutamine stress echocardiography (EDOB) and evaluate its usefulness for improving the diagnostic power of EDOB.
