Background: The hair follicle is a unique structure, one of the most dynamic structures in mammalians, which can reproduce in every new cycle all the mechanism involved in its fetal development. Although a lot of research has been made about the human hair follicle much less has been discovered about the importance of the cytokeratins (CKs) in its development.

Purpose: In order to describe epidemiological and pathological features of penile cancer in a high-risk area of Brazil.

Methods: We reviewed the experience (378 patients from 1997 to 2007) of Hospital Aristides Maltez from Salvador, Bahia-the main institution in the state which provides oncologic treatment for penile cancer in the public health system.

Results: The present series showed a high rate (17 %) of patients less than 40 years at the time of diagnosis.

A male dark-skinned patient, 33 years old, complaining of a 1-month history of skin lesions in the abdominal region and high fever was admitted to the clinical ward. There were ulcerated and coalescent skin lesions all over the body but sparing the palmar region. The culture of material obtained from the penile lesions was positive for Staphylococcus aureus.
