Conformational and ionization equilibria of flexible weak polyelectrolytes (PEs) are, in general, strongly coupled. In this article, we analyze the effect of averaging over (or "contracting") the conformational degrees of freedom so that the original flexible molecule is replaced by an effective rigid object with the same ionization properties. As a result, one obtains the so-called Site Binding (SB) model, much easier to treat both theoretically and computationally, and extensively used to characterize the ionization properties of PE.

Introduction: Psychiatric conditions are one of the leading non-battle injury diseases resulting in medical evacuation (MEDEVAC) from combat environments. The challenge of limited MEDEVAC capability necessitating prolonged field care in future large-scale combat operations must be addressed. Therefore, a robust program is needed to address frontline care of behavioral health (BH), maximizing service members returning to duty and minimizing MEDEVAC.

Background: Immunosuppressed (IS) patients, particularly solid organ transplant recipients and those on immunosuppressive therapy, face a higher incidence and recurrence of nonmelanoma skin cancers (NMSC), including basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). Mohs micrographic surgery (MMS) is the preferred treatment for high-risk NMSC due to its high cure rate and margin examination capabilities. However, IS patients may experience more complications, such as surgical site infections, and a greater risk of recurrence, making their outcomes a subject of interest.

Background: The coronavirus disease 2019 (COVID-19) pandemic has impacted healthcare guidelines and modalities of patient consultation worldwide. The frequent cycles of quarantine confinement in Chile have caused mobility restrictions for patients and physicians, forcing the Hospital Las Higueras de Talcahuano (HHT) to replace the assisted televisit modality with a more classic televisit program. Here we have described if this change in televisit modality and type of outpatient may have impacted patients' satisfaction.

Article Synopsis
  • A new multitarget stool DNA test was developed to enhance colorectal cancer screening by improving specificity and assessing both DNA markers and hemoglobin levels.
  • In a study involving over 20,000 adults aged 40 and older undergoing screening colonoscopy, the test’s sensitivity for colorectal cancer was found to be 93.9% and its specificity for identifying advanced neoplasia was 90.6%.
  • Compared to a standard fecal immunochemical test (FIT), the multitarget test showed significantly higher sensitivity for detecting colorectal cancer and advanced precancerous lesions, although the FIT had slightly better specificity for advanced neoplasia.

A consistent association has been observed between internet addiction and symptoms of social anxiety. However, there is a lack of empirical research that delves into potential explanations for this relationship and its directionality, making it difficult to translate findings into development of interventions for social anxiety that account for technology-related behaviors. The present study aimed to evaluate the longitudinal dynamics between internet addiction, symptoms of social anxiety, avoidance of social interactions, and using the internet to cope with loneliness.

  • Ecuador experienced significant COVID-19 mortality in 2020, despite early implementation of non-pharmaceutical interventions (NPIs), highlighting challenges in high-mobility informal labor settings.
  • A study analyzed respiratory samples from 1950 patients during the pandemic, finding correlations between lower NPI stringency and increased detection of multiple respiratory viruses, including SARS-CoV-2.
  • Phylogenomic analyses showed numerous independent introductions of SARS-CoV-2 in early 2020, with NPIs negatively correlating with the variety of circulating virus strains, emphasizing the importance of effective interventions in controlling outbreaks, particularly in resource-limited contexts.

Background: Despite the evidence-based guidelines promoted by the Global Initiative for Chronic Obstructive Lung Disease (GOLD), the overuse of prescription drugs to manage COPD, particularly inhaled corticosteroids (ICS), remains a persistent challenge. In this real-world study, we evaluated how patients with COPD were divided into ABCD groups based on the 2017 GOLD guidelines, determined the rate of adherence to the GOLD treatment recommendations, described the rate of ICS usage, and determined the rate of triple therapy (TT) prescription.

Methods: The charts of 2291 patients diagnosed with COPD were retrospectively analyzed, of which 1438 matched the eligibility criteria.

Adult lungs present high cellular plasticity against stress and injury, mobilizing stem/progenitor populations from conducting airways to maintain tissue homeostasis and gas exchange in alveolar spaces. With aging, pulmonary functional and structural deterioration occurs, mainly in pathological conditions, which is associated with impaired stem cell activity and increased senescence in mice. However, the impact of these processes underlying lung physiopathology in relation to aging has not been explored in humans.

In this paper, we attempted to elaborate on an atlas of the head of the rhinoceros iguana, applying modern imaging techniques such as CT and MRI. Furthermore, by combining the images acquired through these techniques with macroscopic anatomical sections, we obtained an adequate description of the relevant structures that form the head of this species. This anatomical information could provide a valuable diagnostic tool for the clinical evaluation of different pathological processes in iguanas such as abscesses and osteodystrophy secondary to nutrient imbalances, skull malformations, fractures, and neoplasia.

The electrostatic potential (EP) generated by the protein α-lactoalbumin in the presence of added salt is computed as a thermal average at a given point in space. With this aim, constant pH Monte Carlo (MC) simulations are performed within the primitive model, namely, the solvent is treated as a continuum dielectric. The study of the thermal and spatial fluctuations of the EP reveals that they are remarkably high inside the protein.

Article Synopsis
  • The study examines primary plasma cell leukemia (PCL), previously defined by ≥ 20% circulating plasma cells, now suggested to be diagnosed by a lower threshold of ≥ 2% CTCs, indicating it might just be ultra-high-risk multiple myeloma (MM).
  • Researchers measured CTC levels in 395 newly diagnosed transplant-ineligible MM patients and found that those with 2%-20% CTCs had poorer survival outcomes compared to those with < 2%, emphasizing the clinical relevance of a lower cutoff.
  • The findings support using the 2% cutoff as a potential biomarker for identifying hidden primary PCL and advocate for flow cytometry assessment during MM diagnosis to improve patient stratification and treatment decisions.
