Cerebral cavernous malformations (CCMs) are hemorrhagic vascular disorders with varied clinical and radiological presentations, occurring sporadically due to MAP3K3 or PIK3CA mutations or through inherited germline mutations of CCM genes. This study aimed to clarify the clinical, genetic, and pathological features of CCMs using a multicenter cohort across three Chinese centers. We analyzed 290 surgical specimens from symptomatic CCM patients, utilizing whole-exome sequencing, droplet digital PCR, and targeted panel sequencing, alongside immunohistology to examine genotypic and phenotypic differences.

Article Synopsis
  • - The study evaluates the effects of unilateral combined bypass surgery on pediatric patients with moyamoya disease (MMD), focusing on disease progression in the non-operated side and identifying potential risk factors.
  • - Out of 27 patients, 40.7% experienced disease progression on the non-operated side after surgery, indicated by an increase in moyamoya vessel grade and Suzuki stage, despite minimal changes in regional cerebral blood flow (rCBF).
  • - The findings suggest that unilateral bypass surgery may accelerate radiological progression in MMD patients on the untreated side, particularly in those with more advanced disease prior to surgery.

  • Spinal cord diffuse midline glioma (DMG) with H3 K27-alteration is a serious pediatric brain tumor that usually has a poor prognosis, and this study reports a unique case with unusual early symptoms.
  • A 7-year-old boy showed signs of increased pressure in the brain but didn't exhibit typical spinal cord symptoms; imaging tests later revealed an intraspinal tumor.
  • The findings suggest that in similar cases where brain pressure symptoms occur without clear causes, checking the spinal cord might help identify underlying issues like DMG.

Introduction: Type I Chiari malformation (CM-I) is a common congenital malformation, often accompanied by syringomyelia. Obstruction of CSF circulation in the foramen magnum is the reason for the formation of the syringomyelia. Fourth ventricle stenting (FVS) could ensure the CSF flow from the fourth ventricle, which might represent an effective approach to achieve syringomyelia relief.
