Cochlear Meniere's: A Distinct Clinical Entity With Isolated Cochlear Hydrops on High-Resolution MRI?

High Resolution Three-Dimensional Delayed Contrast MRI Detects Endolymphatic Hydrops in Patients With Vertigo and Vestibular Schwannoma.

Objective: Advances in high resolution magnetic resonance imaging (MRI) have enabled the detection of endolymphatic hydrops (EH), a pathological ballooning of the endolymphatic fluid system, known to be associated with Menière's disease. When a patient has a known diagnosis of vestibular schwannoma and develops recurrent episodic vertigo spells, many surgeons recommend surgical intervention, attributing the vestibular symptoms to the vestibular schwannoma. The aim of this study is to evaluate the clinical outcome in patients with vestibular schwannoma and EH, treated medically, for recurrent spells of vertigo.

Histopathological and ultrastructural analysis of vestibular endorgans in Meniere's disease reveals basement membrane pathology.

Background: We report the systematic analysis of the ultrastructural and cytological histopathology of vestibular endorgans acquired from labyrinthectomy in Meniere's disease.

Methods: 17 subjects with intractable Meniere's disease and ipsilateral non-serviceable hearing presenting to the Neurotology Clinic from 1997 to 2006 who chose ablative labyrinthectomy (average age = 62 years; range 29-83 years) participated. The average duration of symptoms prior to surgery was 7 years (range 1-20 years).

A longitudinal study of oculomotor function in normal older people.

Cross-sectional studies have found declines in most measures of oculomotor function in older subjects compared to young controls, but no prior study has followed the same subjects over time. We measured saccade peak velocity, saccade delay time, smooth pursuit, optokinetic nystagmus (OKN), visual-vestibular-ocular-reflex (VVOR), fixation suppression of the vestibulo-ocular reflex (VOR-fix), and the vestibulo-ocular reflex in 53 subjects (older than 75 years) able to complete at least 9 yearly evaluations. In addition at each visit all patients underwent a complete history and examination, a gait and balance assessment, mini-mental status evaluation, and visual acuity testing.
