Psychosis associated with anti-N-methyl-D-aspartate receptor antibodies.

We describe the eighth case study of a female diagnosed with anti-N-methyl-D-aspartate receptor encephalitis without an identified tumor who presented with floridly psychotic symptoms following a 2-week prodromal phase with new-onset headaches and presyncopal episodes. While hospitalized, the patient had seizures, autonomic dysfunction, involuntary movements, and a decline in mental status. A subsequent assay was positive for anti-N-methyl-D-aspartate receptor receptor antibodies.

Prevalence and epidemiology of pathological gambling at Naval Medical Center Portsmouth psychiatry clinic.

Objective: Gambling has exploded in popularity, but pathological gambling (PG) is infrequently diagnosed. The objectives of this study were to calculate the prevalence of PG in a psychiatry clinic, to determine whether PG is underdiagnosed, and to analyze risk factors for PG.

Methods: A survey was completed by 584 outpatients presenting to the Naval Medical Center Portsmouth psychiatry clinic over 6 months.

Anxiety and abnormal eating behaviors associated with cyclical readiness testing in a naval hospital active duty population.

Studies of abnormal eating behaviors in active duty military personal have found rates similar to or higher than the general population. We have reviewed these studies and extended the research to examine abnormal eating behaviors in a heterogeneous population at a major military medical center. We found high rates of body dissatisfaction, abnormal eating behaviors, and worry about passing the semiannual personal fitness assessment in both men and women.

Transient ischemic attack reported with paroxetine use.

Objective: To report the occurrence of a transient ischemic attack (TIA) temporally related to the initiation of paroxetine.

Case Summary: A 57-year-old white man with a history of intermittent atrial fibrillation and hypercholesterolemia developed slurred speech and a facial droop 3 days after starting paroxetine. He was diagnosed with a TIA, hospitalized, and given anticoagulation treatment.

Psychosis related to ephedra-containing herbal supplement use.

Ephedra, a psychoactive substance with stimulant properties, is found in many herbal products. Often perceived by the lay public as benign, the potential health-related dangers of using these products are beginning to be recognized. We review four cases associated with ephedra-containing herbal products and report three additional cases.

Crisis intervention in a military population: a comparison of inpatient hospitalization and a day treatment program.

We describe transition from an inpatient psychiatric crisis intervention model to an outpatient program for nonpsychotic young adult active duty males presenting in crisis. We reviewed 300 medical records of comparable patients from each program. Demographics, clinical outcomes, and cost analysis are presented.
