Introduction: Recent studies showed that balanced opioid-free anesthesia is feasible and desirable in several surgical settings. However, in thoracic surgery, scientific evidence is still lacking. Thus, we conducted the first systematic review and meta-analysis of opioid-free anesthesia in this field.

Objective: There are concerns of a high barotrauma rate in coronavirus disease 2019 patients with acute respiratory distress syndrome receiving invasive mechanical ventilation. However, a few studies were published, and reported rates were highly variable. We performed a systematic literature review to identify rates of barotrauma, pneumothorax, and pneumomediastinum in coronavirus disease 2019 acute respiratory distress syndrome patients receiving invasive mechanical ventilation.

Article Synopsis
  • A study in Milan examined acute kidney injury (AKI) and the use of continuous renal replacement therapy (CRRT) in COVID-19 patients on mechanical ventilation, finding that 75% developed AKI and nearly 18% required CRRT.
  • Most AKI cases were mild (stage 1), but there was a significant mortality risk, with 38.9% of the AKI patients and 52.9% of those receiving CRRT dying during their hospital stay.
  • The research highlights that older patients and those with pre-existing chronic kidney disease (CKD) were more likely to develop AKI or need CRRT, indicating higher vulnerability in this group.

Systemic infusion of unfractionated heparin (UFH) is the standard anticoagulation technique for continuous renal replacement therapy (CRRT) during extracorporeal membrane oxygenation (ECMO), but often fails to avoid CRRT circuit clotting. The aim of this study was to assess, in patients undergoing CRRT during venovenous ECMO (vv-ECMO), the efficacy and safety of adding regional citrate anticoagulation (RCA) for CRRT circuit anticoagulation (RCA + UFH group) compared with the sole systemic heparin anticoagulation (UFH group). We performed a retrospective chart review (2009-2018) of patients treated with CRRT during ECMO.
