Lower Eyelid Blepharoplasty With Volume Preservation Using the Skin Flap.

Lower blepharoplasty is one of the most commonly performed facial plastic surgeries and can be approached through many different techniques to improve perceived aging and fatigue due to prominent eyelid fat pads, deep tear troughs, loose eyelid skin, and global periorbital deflation. The technique discussed herein is a personal approach to lower eyelid rejuvenation surgery by the senior author, focusing on volume preservation with conservative resection and transposition of lower eyelid fat pads, muscle preservation, and microfat grafting. The authors discuss a series of 80 patients with case examples and a technique video.

Prevention of Umbilical Sagging After Medium Definition Liposuction.

Background: The loss of the umbilical vertical axis, causing a horizontal shape deformity after liposuction, is a current aesthetic issue. The use of energy devices, such as LASER and VASER, has been advocated as an option for improving skin retraction, but no data are available on the prevention of umbilical sagging.

Objectives: The authors sought to describe a technique for preventing umbilical deformities after medium definition liposuction employing suction-assisted liposuction.

Program Director Survey of Aesthetic Plastic Surgery Fellowships in the United States: Current State-of-the-Art.

Background: Aesthetic surgery is a core component of plastic and reconstructive surgery. In 2014, the Accreditation Council for Graduate Medical Education established aesthetic surgery minimums for plastic surgery residents in training. Although many plastic surgery graduates successfully enter practice as an attending plastic surgeon after completion of plastic surgery training, others choose subspecialization.

Arm Contouring: Review and Current Concepts.

Arm contour improvement is a desired goal of the massive weight loss and the aging population who want to maintain the arm aesthetics. Brachioplasty is a growing and safe procedure to improve the arm contour and different approaches and innovative treatment options have been described since the 1930s. This paper reviews the relevant literature in arm contouring including surgical and nonsurgical procedures used to aesthetically improve the arm.

The Circumrotational Technique for Mastopexy.

Numerous combinations of pedicle design and incision patterns have been described for mastopexy, but upper pole volume deficiency, suboptimal shape, or recurrent ptosis are still undesired postoperative findings. The challenges of preventing such outcomes are amplified in the massive weight loss (MWL) patient population, where both the extent of morphologic deformation and alterations in tissue characteristics (ie, a materials failure) can be severe. To correct this problem, we propose a technique that combines breast circumference-reduction with maximal glandular rotation and superomedial repositioning: the circumrotational technique.

Dynamic diagnosis of "fishmouthing" syndrome, an overlooked complication of blepharoplasty.

Background: Dysfunction and/or dehiscence of the lateral canthus is 1 source of symptomatic eyelid closure disorder after blepharoplasty. Because the resulting concentric blinking movement resembles mouth closure in a fish, the name "fishmouthing" syndrome (FS) was given to this condition. Fishmouthing syndrome appears to be an overlooked complication of blepharoplasty.
