Historical data analysis shows that escalation accidents, so-called domino effects, have an important role in disastrous accidents in the chemical and process industries. In this study, an agent-based modeling and simulation approach is proposed to study the propagation of domino effects in the chemical and process industries. Different from the analytical or Monte Carlo simulation approaches, which normally study the domino effect at probabilistic network levels, the agent-based modeling technique explains the domino effects from a bottom-up perspective.

By modeling domino effects in chemical plants as a directed graph, the graph centrality measures such as out-closeness and betweenness scores can be used to identify the installations playing a key role in initiating and propagating potential domino effects. It is demonstrated that active fire protection of installations with the highest out-closeness score and passive fire protection of installations with the highest betweenness score are the most effective strategies for reducing the vulnerability of chemical plants to fire-induced domino effects.

Simplified models for the estimation of the vessel time to failure (ttf) with respect to the radiation intensity on the vessel shell were obtained using a multi-level approach to the analysis of vessel wall failure under different fire conditions. Each vessel "time to failure" calculated by this approach for the specific fire scenario of concern was compared to a reference time required for effective mitigation actions and related to the escalation probability.

Experimental data available for small scale vessels fully engulfed by a fire were extended to real scale road and rail tankers through a finite elements model. The results of mathematical simulations of real scale fire engulfment scenarios that may follow accidents involving LPG tankers proved the effectiveness of the thermal protections in preventing the "fired" BLEVE (Boiling Liquid Expanding Vapour Explosion) scenario.

A specific test protocol was defined to enhance reproducibility of experimental tests. The geometrical characteristics of the test tanks were selected in order to obtain shell stresses similar to those present in full-size road tankers complying to ADR standards.

Reference schemes were defined for alternative processes proposed for hydrogen storage, and several storage potentialities were considered.
