Invasive pneumococcal disease (IPD) in the elderly is an important public health issue due to the increased proportion of this population in many countries including Brazil. Streptococcus pneumoniae is a rare cause of appendicitis.We report apneumococcal appendicitis with secondary peritonitis in a human immunodeficiency virus–positive adult, with favorable outcome after surgery and antibiotic therapy.
