Article Synopsis
  • Obesity negatively affects the prognosis of COVID-19 patients with acute respiratory distress syndrome (ARDS), particularly those requiring veno-venous extracorporeal membrane oxygenation (V-V ECMO).
  • An observational study analyzed data from 354 patients on V-V ECMO across 82 institutions, finding no significant differences in characteristics between high BMI (over 40 kg/m²) and lower BMI groups, though the high BMI group was younger and had a lower severity score.
  • Survival analysis revealed that both older age and higher BMI are associated with increased risk of death in these patients, with no specific cutoff BMI identified; rather, the risk rises steadily with increasing BMI.

Carfentanil and related fentanyl analogs have been linked to a number of overdose deaths from drug users in several cities across North America. Diagnosis of carfentanil exposure requires a very high index of clinical suspicion, especially because available laboratory narcotic screens do not detect this agent. We describe a 34-year-old man admitted with depressed level of consciousness and in respiratory failure after recreational exposure to a white powder later inferred to contain carfentanil.

An elevation of cardiac injury markers including creatinine kinase (CK), myoglobin (Myo) and cardiac troponin T (cTnT) has been observed in elite athletes following strenuous exercise. The mechanism and significance of this observation however have not been fully elucidated. The goals of this study were: 1) to determine whether these changes in biomarkers also occur in a large, heterogeneous group of non-elite athletes; and 2) to identify possible clinical or biochemical associations.
