Treatment of chronic pain disease is a scientific and clinical challenge encountered in all branches of medicine. Essential trigeminal neuralgia and chronic pain situations are noted for their exceptionally marked severity and also for their psychic and social consequences. It is not rare for a primarily somatic pain syndrome to develop into a painful disease in its own right, which is highly refractory to treatment.

In a clinical statistical evaluation of patients seen in our hospital between 1967 and 1987 a total of 25 cases of facial asymmetry are assessed. The importance of CT scans for diagnosis and treatment planning is emphasized. The histochemical analysis of native cryostat hard bone tissue sections proves the existence of atypical bone remodelling processes in the inner bone surface late in the 3rd decade of life.

By means of an investigation on 7 patients from 3 families views are expressed on the problems of the basal cell-naevus-syndrome. The typical clinical signs appear preferentially and as first-symptoms in the jaw-bone as solitary and later developing multiple cysts. Because these first symptoms of the syndrome cause secondary, dentitio tarda (belated dentition) and displacement of the teeth, the dentist or the maxillo-facial-surgeon has a special responsibility for early diagnosis of this disease.

Despite of respiratory infections, otitis media, sinusitis, aphtosis and exogenous allergy the first detection of IgA deficiency was delayed til adulthood (mean age 42 years). A case of a 22 years old women with complicated fistula after molar tooth extraction showed the problems of dentistry in such diagnosis.
