Platypnea orthodeoxia is a rare disorder characterized by dyspnea and arterial desaturation, exacerbated by the upright position and relieved when the subject is recumbent. We report the case of a 79-year old woman admitted to hospital with dyspnea who was thought to have restrictive ventilatory impairment due to osteoporosis and severe kyphosis. Interestingly, the dyspnea was aggravated in the upright position, whereas the symptoms improved in the supine position.

We report the case of a patient with congenital sick sinus syndrome complicated by atrial fibrillation and embolic stroke 23 years after the initial diagnosis, at the age of 34 years. Treatment with a dual-chamber pacemaker and oral anticoagulation were initiated; further follow-up was uneventful but pacemaker diagnostics constantly documented asymptomatic recurrences of paroxysmal atrial fibrillation.

Background: Critical sites within reentry circuits of postinfarction ventricular tachycardia (VT) were identified during sinus rhythm (SR) and VT to determine whether electrogram characteristics during SR may be helpful in identifying successful ablation sites.

Methods: In 33 patients (mean age 67 +/- 11 yrs) with prior infarction, mapping and radiofrequency (RF) catheter ablation of 57 hemodynamically-tolerated VT's (cycle length 478 +/- 96) were performed. The morphologies of electrograms (EGM) at sites of concealed entrainment (CE) were compared during SR and VT.
