Article Synopsis
  • The article systematically reviews published clinical prediction rules that estimate the risk of Down syndrome (DS) based on second-trimester ultrasound findings during pregnancy.
  • Ten clinical prediction rules were identified; only three of them have been validated, and a study using the rules on 2216 ultrasound scans showed low agreement among different methods.
  • While these prediction rules can aid in assessing DS risk, their inconsistent results highlight the need for further external validation to determine the most reliable options for clinical practice.

Objective: To review systematically the literature on diagnostic tests and performance of second-trimester sonographic assessment of nasal bone (NB) in identifying fetuses affected by Down syndrome.

Methods: A search of studies involving screening tests for NB evaluation and measurements was carried out in the main international bibliographic databases (MEDLINE, EMBASE and CINAHL). Those considered to be relevant were then subjected to critical reading, following Critical Appraisal Skills Programme (CASP) criteria, by at least three independent observers.
