Article Synopsis
  • Endometriosis affects about 10% of women of reproductive age and leads to issues like chronic pain and dysmenorrhea, impacting overall quality of life.
  • The main treatments focus on pain management and fertility preservation, often using hormonal therapies and assisted reproductive technologies (ART) such as IVF.
  • Recent studies suggest that IAP (inhibitor of apoptotic protein) inhibitors and other compounds like parthenolide may offer new therapeutic options by targeting the inflammatory processes involved in endometriosis.

Problem: What is the effect of SR-16234 (SR), a selective estrogen receptor (ER) modulator, on human endometriotic stromal cells (ESCs)?

Method Of Study: Endometriotic tissues were obtained from 21 patients undergoing laparoscopic surgery for ovarian endometriomas (OEs). Normal eutopic endometrium during the luteal phase was obtained from 18 patients without endometriosis. ESCs isolated from OEs and normal eutopic endometrial stromal cells (NESCs) were cultured with SR and subsequently exposed to tumor necrosis factor (TNF)-α.

Objective: To evaluate the effects of a P2X4 receptor (P2X4R)-specific antagonist on murine endometriotic-like lesions and human endometriotic stromal cells.

Design: Experimental study using an in vivo mouse endometriosis model and in vitro primary culture of human endometriotic stromal cells. NC-2600, an antagonist of the P2X4 ionotropic ATP receptor (P2X4R), was orally administered to the mice and cells.

Aim: The chronic lack of obstetrics-gynecology (Ob/Gyn) doctors is a serious problem to be tackled in Japan. To address this issue, it is necessary for medical students to be interested in working as an Ob/Gyn doctor and more junior residents need to specialize in Ob/Gyn. This study aims to identify the preferences concerning working conditions among students and discusses what information should be provided and what conditions should be changed.

Article Synopsis
  • - The Japan Society for Endoscopic Surgery (JSES) conducted its 16th National Survey for 2020 and 2021, highlighting the growth of laparoscopic procedures in Japan since its introduction in 1990.
  • - In 2021, over 290,000 patients underwent various endoscopic surgeries, with the majority being abdominal surgeries; however, COVID-19 negatively affected the number of surgeries performed in 2020.
  • - Despite the pandemic's impact, robot-assisted surgeries gained popularity, and the overall rate of complications remained stable, suggesting that these procedures were performed safely even during challenging times.

Introduction: Hybrid total laparoscopic hysterectomy combines conventional laparoscopic surgery and robot-assisted devices: the camera and assistant forceps are operated by a robotic device, whereas the surgeon performs laparoscopic procedures, enabling surgery with a completely fixed field of view and significantly reducing errors in forceps grasping and needle misalignment. Here, we examined whether using two arms of the Hugo™ robot-assisted surgery system, one for the camera and one for the assistant, would improve surgical accuracy compared with conventional total laparoscopic hysterectomy.

Materials And Surgical Technique: The surgical system reduced surgeon errors in grasping the forceps during training and stabilized forceps operation.

Background: Amphoterin-induced gene and open reading frame 2 (AMIGO2) have been reported to be related to the prognosis of colorectal, gastric, and cervical cancer. However, their association with ovarian cancer remains unclear. This study aimed to elucidate the role of AMIGO2 in ovarian cancer.

Purpose: Neoangiogenesis is necessary for adhesion and invasiveness of endometriotic lesions in women affected by endometriosis. Vascular endothelial growth factor (VEGF) is one of the main components of angiogenesis and is part of the major pathway tissue factor (TF)-protease activated receptor-2 (PAR-2)-VEGF that leads to neoangiogenesis. Specificity protein 1 (SP1) is a transcriptional factor that has recently been studied for its crucial role in angiogenesis via a specific pathway.

Few studies have compared the efficacy of robot-assisted, laparoscopic, and open surgeries for endometrial cancer. When considering the position of robotic surgery in Japan, it was necessary to determine whether it was effective or not. We aimed to compare the efficacy and safety of these three types of surgeries for early-stage endometrial cancer.

Background: Endometriosis-related pain encompassing dysmenorrhea, dyspareunia, and chronic pelvic pain, reduces the quality of life in premenopausal women. Although treatment options for endometriosis alleviate this pain, approximately one-third of women still experience pain even after receiving treatment, indicating the need for novel approaches to pain relief in those women. The Angel Touch device (AT-04) is a portable magnetic fields irradiation device that incorporates a combination of mixed alternative magnetic fields at 2 kHz and 83.

This study describes the first reported treatment of a benign gynecological disease with the Medtronic's Hugo™ RAS System, the first robot-assisted device used in the field of gynecology in Japan. Patients who underwent robot-assisted total laparoscopic hysterectomy with the Hugo™ RAS System at the Tottori University Hospital between March 2023 and June 2023 were included in this study. In the first case, the patient was in her 40 s and had uterine fibroids.

Malignant pericardial effusion is an uncommon metastatic manifestation of ovarian carcinoma. Few cases of ovarian serous carcinoma have been previously reported. Ovarian clear cell carcinoma is the second most common histologic subtype in East Asian countries and is a relatively rare in Western countries.

Article Synopsis
  • The study investigates the relationship between age, ovarian endometrioma size, and pain levels in women with endometriosis, finding that age affects pain scores but cyst size does not.
  • Based on data from 491 patients across six clinical trials, the study reported average scores for dysmenorrhea, menstrual pain, and pelvic pain, with pelvic pain decreasing as age increased.
  • Limitations include a lack of data on participants' previous treatments and the challenge of accurately assessing pain due to psychological factors.

Mayer-Rokitansky-Küster-Hauser syndrome (MRKHS) is a rare congenital anomaly of the genital tract. Since the secretion of sex hormones from the ovaries is preserved, leiomyomas and adenomyomas, which are estrogen-dependent diseases, may develop from the uterine remnant. In contrast, patients with myotonic dystrophy type 1 (DM1), the most common dystrophy in adults, are considered to be at high risk for benign tumors of the female reproductive system, such as uterine leiomyomas and ovarian cysts.

Objective: Adenomyosis is a gynecologic disorder characterized by symptoms of dysmenorrhea, abnormal uterine bleeding, and infertility. This study aimed to analyze the expression profiles of key inflammatory cytokines in the endometrium with adenomyosis and their involvement in epithelial-mesenchymal transition (EMT).

Study Design: Endometrial tissues collected from premenopausal women with (n = 3) or without (n = 3) adenomyosis during the secretory phase were subjected to DNA array analysis to examine inflammatory cytokines.

Article Synopsis
  • This study compared robotic-assisted hysterectomy (RAH) and total laparoscopic hysterectomy (TLH) for treating benign gynecological diseases at Tottori University Hospital from January 2018 to May 2022.
  • It included 40 patients in the RAH group and 44 in the TLH group, evaluating metrics like total operative time and learning curve.
  • The results showed that after about 30 RAH surgeries, the total operative time was similar to TLH, and after about 10 surgeries, the operative time for RAH was also comparable to TLH.
