Background: Companion diagnostic tests play a crucial role in guiding treatment decisions for patients with non-small cell lung cancer (NSCLC). The Oncomine Dx Target Test (ODxTT) Multi-CDx System has emerged as a prominent companion diagnostic method. However, its efficacy in detecting driver gene mutations, particularly rare mutations, warrants investigation.

Article Synopsis
  • - The study investigates long COVID symptoms among 935 patients in Japan, categorizing them into five distinct clusters based on various symptom presentations and their impact on quality of life (QOL).
  • - The clusters include: numerous symptoms affecting multiple organs, minimal symptoms, taste and smell disorders, fatigue with psychological symptoms, and fatigue with respiratory issues, showcasing a diverse range of long COVID experiences.
  • - Findings highlight the need for personalized treatment approaches, as the severely affected patients (mostly elderly in cluster 1) experienced the greatest decline in QOL, while other clusters showed different symptom trends and demographic profiles.

Article Synopsis
  • * Data from 1200 enrolled patients showed a decline in symptoms over time, from 93.9% during hospitalization to 33.0% at 12 months, with those experience long COVID symptoms reporting lower quality of life and higher anxiety and depression.
  • * Identified risk factors for long COVID included being female, middle-aged, requiring oxygen, and being in critical condition during hospitalization, providing valuable insights for future research in Japan.

Article Synopsis
  • * After vaccination, 13.5% of patients experienced symptom improvement, 3.6% had worsening symptoms, while 82.9% saw no change in their long COVID symptoms.
  • * The most common symptoms that improved post-vaccination were dyspnea and alopecia, but many symptoms like sleep disturbance and myalgia persisted, indicating that vaccination did not significantly impact long COVID for most patients.

We report the first case of a patient with non-small cell lung cancer (NSCLC) with malignant pleural effusion (MPE) who achieved disease- and treatment-free survival for nearly 10 years. A 50-year-old man was diagnosed with NSCLC with MPE and underwent chemotherapy and salvage thoracic surgery. The patient received chemotherapy with cisplatin, pemetrexed, and bevacizumab, and a partial response was achieved.

Article Synopsis
  • * Researchers analyzed RNA-seq data from 465 blood samples, uncovering 1169 expression quantitative trait loci (eQTLs) and 1549 splice QTLs (sQTLs) linked to COVID-19 severity, including immune-related expressions.
  • * The study highlights the impact of disease severity on gene expression, identifying specific eQTLs that interact with COVID-19 phenotypes, and provides an extensive resource for understanding gene regulation in response to the virus.

Article Synopsis
  • Researchers conducted a genome-wide association study (GWAS) with 2,393 COVID-19 patients and 3,289 controls in Japan, identifying a specific genetic variant (rs60200309-A) on chromosome 5 linked to severe cases in individuals under 65.
  • The variant is more common in East Asians and associated with reduced expression of the DOCK2 gene, which was found to be lower in severe COVID-19 cases, particularly in non-classical monocytes.
  • Additionally, inhibiting DOCK2 in hamsters worsened pneumonia symptoms, indicating its potential as a biomarker and therapeutic target for severe COVID-19.

While small cell lung cancer (SCLC) has been treated as a single disease historically, recent studies have suggested that SCLC can be classified into molecular subtypes based on the expression of lineage transcription factors such as achaete-scute homolog 1 (ASCL1), neurogenic differentiation factor 1 (NEUROD1), POU domain class 2 transcription factor 3 (POU2F3) and transcriptional coactivator YAP1 (YAP1). These transcription factor-based subtypes may be specifically targeted in therapy, and recent studies have suggested that the SCLC subtypes represent different stages of dynamic evolution of SCLC rather than independent diseases. Nevertheless, evidence of shift in neuroendocrine differentiation during SCLC evolution has been lacking in the clinical setting.

Ruxolitinib, a Janus kinase inhibitor, improves symptoms in patients with myelofibrosis. However, its association with the development of opportunistic infections has been a concern. We herein report a 71-year-old man with primary myelofibrosis who developed disseminated tuberculosis and concurrent disseminated cryptococcosis during ruxolitinib treatment.

Reverse transcription-quantitative PCR (RT-qPCR)-based tests are widely used to diagnose coronavirus disease 2019 (COVID-19). As a result that these tests cannot be done in local clinics where RT-qPCR testing capability is lacking, rapid antigen tests (RATs) for COVID-19 based on lateral flow immunoassays are used for rapid diagnosis. However, their sensitivity compared with each other and with RT-qPCR and infectious virus isolation has not been examined.

A 70-year-old woman with rheumatoid arthritis undergoing methotrexate (MTX) treatment presented with dyspnea and a subfever. Computed tomography (CT) revealed a diffuse minimal ground-glass appearance in both lungs and splenomegaly. The gallium scintigram showed a diffuse, mild uptake in both lungs and the spleen.
