The objective is to impart an understanding of the potentials and limitations of various restorative therapies to enable clinicians to determine the circumstances under which they should select a particular option among the available therapies in the treatment of compromised posterior teeth.

When contemplating removal of a premolar or molar and its immediate replacement with an implant-supported prosthesis, the feasibility of other potentially less "aggressive" treatment approaches must first be considered. Treatment decisions must be made in the context of an appropriate definition of therapeutic success. Replacement of hopeless mandibular molars with implant restorations has undergone significant evolution since the introduction of osseointegrated implants some 30 years ago.

Patients are more aware of dental esthetics than ever before and demand more than just a replacement of a missing tooth. The selection of the type of provisional restoration can significantly influence esthetics during the period of implant integration and soft tissue healing. A restoration that preserves or helps to regenerate interdental papillae has always been a challenge for both experienced and inexperienced practitioners.
