We report a case of a 31-year-old woman with 2 previous caesarean deliveries who presented a diagnostic dilemma. She underwent caesarean section where she was found to have a uterine scar dehiscence with an adherent placenta and required a hysterectomy. While individually representing high risk to the mother and/or fetus, the combination in this case protected the fetus.

Background: The majority of women have spontaneous vaginal births, but some women need assistance in the second stage with delivery of the baby, using either the obstetric forceps or vacuum extraction. Rates of instrumental vaginal delivery range from 5% to 20% of all births in industrialised countries. The majority of instrumental vaginal deliveries are conducted in the delivery room, but in a small proportion (2% to 5%), a trial of instrumental vaginal delivery is conducted in theatre with preparations made for proceeding to caesarean section.

Objective: To assess trends in twinning over four decades using a population-based registry.
Design: Ecological study to conduct trend analysis of twin pregnancies in a geographically defined area over 40 years.
Setting: All pregnancies in the Cardiff and Vale of Glamorgan area of South Wales from 1965 to 2004, as recorded in the Cardiff Birth Survey (CBS) database.
Objective: To describe the perspectives and experiences of women in their use of antenatal care and in their reasoning on specific antenatal care routines.
Design: Two qualitative methods including focus group discussions and individual interviews were combined.
Setting: : a rural district in Zimbabwe where a randomised control trial had been undertaken to introduce a new antenatal care package.
