Undescended testis and testicular torsion represent two frequent andrological diseases that affect the pediatric age. Despite these testicular disorders having different causes, they both negatively influence fertility in adulthood mainly due to the accumulation of reactive oxygen species (ROS), which represents the primary molecular damage underlying their long-term effects. The gold standard of treatment for both pathologies is surgery; however, it cannot guarantee an optimal fertility outcome in all clinical cases, underscoring the need to identify effective adjuvant therapies that may target the augmented ROS levels.

Background: Hao-Fountain syndrome is a neurodevelopmental disorder characterized by global developmental delay, variably impaired intellectual development with significant speech delay and, in some males patients, it has been reported an association with hypogonadism. At present less than 50 cases are reported in literature.

Case Presentation: We report a case of this rare syndrome in a young female with isolated tubal torsion; our patients had different hospitalizations without treatment but during the last episode we decide to perform an abdominal surgical explortion.

Purpose: The treatment and long term clinical outcomes of anorectal malformations (ARM) in children have always been the focus of pediatric surgeons. This study aimed at reporting our experience as far as long-term follow-up of ARM in children is concern.

Methods: We enrolled patients treated between 1999 and 2019, and established selection criteria to choose appropriate subjects.

Background: In recent years, evolution of surgery has led to laparoscopy and then to single port surgery. In pediatric age, few papers have been published about single port procedures; in particular, no one has described the use of the Octoport device (Frankenman International Ltd., Suzhou, China).

Article Synopsis
  • Testicular torsion typically affects young males under 25 and requires surgical intervention, but predicting the functionality of the preserved testicle remains a challenge.
  • Through in vitro culture of gubernacular tissue linked to the testicle, researchers established a method to assess the health of the organ and its potential response to chorionic gonadotropin (hCG), a therapy used in andrology.
  • Findings showed that gubernacular cells can indicate testicular health, particularly as those from patients with severe tissue damage failed to grow, while the presence of luteinizing hormone receptors suggested hCG could enhance cell growth and vascularization post-surgery.

Background: Ongoing innovation in laparoscopy lead pediatric surgeons to consider a mini-invasive approach for inguinal hernia correction. We review cases and evolution of surgical technique in a pediatric center.

Methods: A retrospective study included patients that underwent inguinal hernia repair between 01/01/2011 and 30/06/2019.

At present, there is still a lack of attention to male infertility and fertility impairment. Indeed, the pathologies affecting the reproductive area in man are derived from anatomical or functional alterations of neuroendocrine system; thus, the study of these dysfunctions is necessary for a correct aetiopathogenetic and therapeutic framing of infertile patients. In this article, we underline the importance of the study of the molecular mechanisms regulated by the most common therapy used to treat infertile men, with the aim to highlight the necessity to avoid the administration of the wrong posology or, even more important, the wrong therapy to the patient.

Article Synopsis
  • Varicocele, phimosis, and undescended testes are common male reproductive health issues in children, with varicocele and undescended testes being key contributors to male infertility.
  • The study analyzed data from 20 years of school screenings for boys aged 11-14, revealing an increase in varicocele diagnosis and a decrease in phimosis cases, with no detected instances of undescended testes during the recent screenings.
  • The findings highlight the importance of regular andrological check-ups during childhood and adolescence to help identify and potentially reduce the incidence of fertility-related conditions.

Background: The influence of sex, socioeconomic status, and other factors on outcomes from acute illness has been found recently with an increasing interest; acute appendicitis is the most frequent urgency in pediatric age. The aim of this study was to show any gender differences.

Methods: We retrospectively analyzed the medical records of consecutive patients who underwent surgical exploration for acute appendicitis.

Varicocele is the primary cause of male infertility and the interests of research about varicocele have changed in the last years. The aim of the study was to analyse how topics of interests about varicocele have changed in the last two decades. Literature about paediatric varicocele between 2000 and 2018 was reviewed, and the subjects of the articles were analysed, selecting the main and secondary topics of each article.

Introduction: This is the report of the first official survey from the Italian Society of Pediatric Surgery (ISPS) to appraise the distribution and organization of bedside surgery in the neonatal intensive care units (NICU) in Italy.

Methods: A questionnaire requesting general data, staff data and workload data of the centers was developed and sent by means of an online cloud-based software instrument to all Italian pediatric surgery Units.

Results: The survey was answered by 34 (65%) out of 52 centers.

Introduction: Testicular torsion is an emergency at any age; the aim of this study is to evaluate the role of mean platelet volume to assess the viability of the testes before surgeryMaterials and methods: We retrospectively analysed the medical records of consecutive patients who underwent surgical exploration for acute scrotal pathology between January 2014 and December 2016 in our institution.

Patients: were divided into two groups (detorsion of testes and orchyectomy); a third group was created as control group. All patients underwent blood exam before surgery; inclusion and exclusion criteria were created.

Unlabelled: Preserving the fertility potential of adolescent patients with varicocele is still the main outcome of pediatric and adolescent urologysts; the aim of this study is to assess the fertility potential of patients treated for varicocele with testicular hypotrophy in absence of post-operative complications, with complete ipsilateral testicular catch-up growth and normal hormonal values.

Methods: The Authors collected data from adolescents with varicocele aged between 10 and 16 years treated at their Institution between September 2010 and September 2015. Inclusion and exclusion criteria were created; all patients were followed clinically and with hormonal tests before and after surgery and then re-evaluated after semen analysis.

  • The study aimed to analyze how pediatric ovarian torsion (OT) is managed in Italy, focusing on recurrence rates and surgical techniques used in girls aged 1-14 from 2004 to 2014.
  • Results showed that the average age for surgery was about 10 years, with various surgical methods employed, including adnexectomy and detorsion; recurrence rates were higher in cases without ovarian masses.
  • The study concluded that while oophorectomy remains common, it is not recommended, and that oophoropexy (OPY) appears to effectively reduce recurrence without negatively affecting ultrasound results after one year.

Article Synopsis
  • This study examines how elastosonography can help assess testicular elasticity to predict damage in patients with varicocele.
  • Patients were divided into three groups: those with untreated varicocele, those treated with the same method, and healthy individuals, all of whom underwent sonoelastography.
  • Results indicated that testes with varicocele were significantly stiffer than normal, and while all testes with hypotrophy received a high elastographic score, not all did show associated symptoms, suggesting that elastosonography is a useful tool in evaluating testicular health.
