Article Synopsis
  • Food allergy (FA) affects around 6-8% of young children, with tree nut and peanut allergies being major causes of severe allergic reactions (anaphylaxis).
  • The oral food challenge (OFC) is the most reliable method for diagnosing food allergies, while skin prick tests (SPTs) and serum specific IgE (sIgE) tests are also commonly used.
  • Evidence on the predictive value of SPTs and sIgE regarding OFC outcomes is inconsistent, indicating that while these tests can sometimes provide insights, OFC is still the definitive diagnosis method for food allergies.

Background: The mechanisms at the basis of depression are still matter of debate, but several studies in the literature suggest common pathways with dementia (genetic predispositions, metabolic and inflammatory mechanisms, neuropathological changes) and other geriatric syndromes.

Aims: To evaluate the role of cortisol (as marker of the HPA, hypothalamus-pituitary-adrenal axis hyperactivity) in elderly subjects with depressive symptoms (by the means of the AGICO, AGIng and COrtisol, study), in relationship to the presence of the major geriatric syndromes.

Methods: The AGICO study enrolled patients from ten Geriatric Units in Italy.

allergy is an emerging pathological condition in both general and pediatric populations with a wide range of symptoms linked to variable molecular patterns, justifying systemic and cross-reactions with other allergens (i.e., latex, pollen, and fruit).

Introduction: Radical cystectomy with pelvic lymph node dissection is the gold standard treatment for non-metastatic muscle-invasive bladder cancer and high-risk non-muscle-invasive bladder cancer. For years, the traditional open surgery approach was the only viable option. The widespread of robotic surgery led to its employment also in radical cystectomy to reduce complication rates and improve functional outcomes.

Cow's milk protein allergy (CMPA) is the most common food allergy (FA) in infancy, affecting approximately 2% of children under 4 years of age. According to recent studies, the increasing prevalence of FAs can be associated with changes in composition and function of gut microbiota or "dysbiosis". Gut microbiota regulation, mediated by probiotics, may modulate the systemic inflammatory and immune responses, influencing the development of allergies, with possible clinical benefits.

Background: In the aging process, the cumulative exposure to stress with increased cortisol levels is considered to be associated to the senescence itself and its related disorders.

Aims: To evaluate the role of cortisol in elderly subjects, with or without dementia, by the means of the AGICO study.

Methods: The AGICO study enrolled patients from ten Geriatric Units in Italy in 2012-2017 (Study Director Prof Paolo Falaschi, S.

Article Synopsis
  • - Fournier's gangrene (FG) is a serious bacterial infection affecting the genital area, primarily seen in men aged 60 and older, particularly those with conditions like diabetes and bowel diseases, with about 1.6 new cases per 100,000 each year.
  • - In a presented case, a 60-year-old man with diabetes and an ileostomy experienced severe symptoms of FG, requiring immediate antibiotic treatment and surgical intervention, leading to a lengthy recovery process that lasted 60 days.
  • - FG carries a significant mortality risk, up to 30%, but treatments such as hyperbaric oxygen therapy may enhance healing and recovery in affected patients.

The hepatitis C virus (HCV) infection is an important public health problem and it is associated with hepatic and extrahepatic manifestations. Autoimmune thyroid diseases are common in HCV infected patients and the standard interferon-based treatment is associated with an increase of the immune-mediated thyroid damage. Recent evidence in the literature analyzed critical points of the mechanisms of thyroid damage, focusing on the balance between the two sides of the interaction: The environment (virus infection with potential cross-reaction) and the host (susceptibility genes with consistent immune response).

Hepatic glycogenosis (HG) in type 1 diabetes is a underrecognized complication. Mauriac firstly described the syndrome characterized by hepatomegaly with altered liver enzymes, growth impairment, delay puberty and Cushingoid features, during childhood. HG in adulthood is characterized by the liver disorder (with circulating aminotransferase increase) in the presence of poor glycemic control (elevation of glycated hemoglobin, HbA1c levels).

Background: Alzheimer's disease (AD) causes considerable distress in caregivers who are continuously required to deal with requests from patients. Coping strategies play a fundamental role in modulating the psychologic impact of the disease, although their role is still debated. The present study aims to evaluate the burden and anxiety experienced by caregivers, the effectiveness of adopted coping strategies, and their relationships with burden and anxiety.
