Article Synopsis
  • The study aimed to analyze the impact of race, sex, comorbidities, and Medicaid status on health outcomes for patients with hard-to-heal venous leg ulcers (VLUs), comparing Medicare-only beneficiaries to Medicare/Medicaid dual-enrollees from October 2015 to October 2019.
  • Using Medicare claims data, the research focused on treatment methods, demographic comparisons, and quantified outcomes like ulcer closure time and quality of life metrics, finding notable differences between the two groups.
  • Results showed that dual-enrollees had a higher burden of comorbidities, greater representation from minority backgrounds, and significantly increased rates of emergency visits and cellulitis compared to Medicare-only enrollees, with early use of certain

Article Synopsis
  • A study assessed children under 18 with antibodies to myelin oligodendrocyte glycoprotein (MOG-Ab), focusing on those with encephalitis who didn’t fit the criteria for acute disseminated encephalitis (ADEM), comparing them to those with ADEM.
  • Out of 235 MOG-Ab positive patients, 33 had encephalitis and 74 had ADEM, with common symptoms including headache, seizures, and fever; 24% had a normal initial brain MRI.
  • Findings indicated that children with encephalitis were generally older, more often admitted to intensive care, and had a delayed start of steroid treatment, suggesting MOG-Ab testing is essential for suspected encephalitis

Objective: To learn about the experiences of people who seek treatment for hard-to-heal wounds, we distributed a nationwide pilot survey, asking questions about the nature of their wound, how it shaped their daily lives, pathways to receiving care and experiences with treatment. The long-term objective is to quantify the journey of patients with hard-to-heal wounds to identify ideal intervention points that will lead to the best outcomes. This article summarises the findings, implications, limitations and suggestions for future research.

Article Synopsis
  • Radiotherapy combined with immune checkpoint blockade (ICB) is being tested in a clinical trial involving oligometastatic cancer patients to see if it can enhance T-cell responses and improve outcomes.
  • In this phase II trial, patients were split into two groups: one receiving a combination of treatments immediately after radiotherapy (arm A) and a control group receiving pembrolizumab alone (arm B).
  • Results showed a higher one-year progression-free survival rate in arm A (10%) compared to arm B (0%), but overall, the study did not meet its main goal of improving long-term survival outcomes.

Background: Paediatric convulsive status epilepticus is the most common neurological emergency presenting to emergency departments. Risks of resultant neurological morbidity and mortality increase with seizure duration. If the seizure fails to stop within defined time-windows, standard care follows an algorithm of stepwise escalation to more intensive treatments, ultimately resorting to induction of general anaesthesia and ventilation.

Objectives: The objective of this study is to assess the concordance and its association with sociocultural background of a four-question survey with accelerometry in a multiethnic adolescent population, regarding sleep components. Based on questions from the Pittsburgh Sleep Quality Index and adapted to a school context, the questionnaire focussed on estimating sleep onset time, wake-up time and sleep duration on both weekdays and weekends. This subjective survey was compared with accelerometry data while also considering the influence of sociocultural factors (sex, place of living, ethnic community and socio-economic status).

Australian providers of mental health services and support for young people include private and public allied health providers, government initiatives (eg, headspace), nongovernment organizations (eg, Kids Helpline), general practitioners (GPs), and the hospital system. Over 20 years of research has established that many young people prefer to seek mental health support online; however, clear client pathways within and between online and offline mental health services are currently lacking. The authors propose a Digital Mental Health Practice model of care for youth to assist with digital mental health service mapping.

Objective: To evaluate the cost-effectiveness of dehydrated human amnion/chorion membrane (DHACM) in Medicare enrolees who developed a venous leg ulcer (VLU).

Method: This economic evaluation used a four-state Markov model to simulate the disease progression of VLUs for patients receiving advanced treatment (AT) with DHACM or no advanced treatment (NAT) over a three-year time horizon from a US Medicare perspective. DHACM treatments were assessed when following parameters for use (FPFU), whereby applications were initiated 30-45 days after the initial VLU diagnosis claim, and reapplications occurred on a weekly to biweekly basis until completion of the treatment episode.

Online communities in health care are well established as technologies for facilitating interactions between patients with common conditions and communication between patients and health professionals. Less well known is how health professionals use these platforms to connect with their peers, particularly for interprofessional education or support. We present early results from our exploratory international mixed methods (survey and interview) study of health professionals' experience of engaging with online communities for interprofessional peer education.

Clinical decision support systems (CDSS) can enhance the safety and quality of patient care, but their benefits are often hampered by low acceptance and use by clinicians in practice. Existing research has explored clinicians' experiences with CDSS in a static nature, with limited consideration of how user needs may change over time. This review aimed to identify the methods used to capture clinicians' acceptance and use of CDSS in hospital settings at different time points following implementation and highlight gaps to inform future work.

Background: Patients with advanced melanoma who progress after treatment with immune checkpoint-inhibitors (ICI) and BRAF-/MEK-inhibitors (if mutated) have no remaining effective treatment options. The presence of CD1c (BDCA-1) and CD141 (BDCA-3) myeloid dendritic cells (myDC) in the tumor microenvironment correlates with pre-existing immune recognition and responsiveness to immune checkpoint blockade. The synthetic saponin-based immune adjuvant AS01 enhances adaptive immunity through the involvement of myDC.

Article Synopsis
  • Lesion resolution is more common in children with myelin-oligodendrocyte glycoprotein antibody-associated disease (MOGAD) compared to those with multiple sclerosis (MS), with 83% of MOGAD patients showing at least one resolved lesion during follow-up, while MS patients rarely experience this.
  • In a study of 200 children (97 with MOGAD and 103 with MS), new symptomatic and asymptomatic lesions were significantly more prevalent in MS than in MOGAD, indicating different disease behaviors.
  • Early intervention with steroids and plasma exchange positively impacted lesion dynamics in MOGAD, highlighting the importance of timely treatment to reduce new lesions and enhance recovery potential.

The effect of age at injury on outcomes after brain injury has long been debated. Many have argued that the greater plasticity of the immature brain aids in its recovery from trauma, but others (notably Donald Hebb) have argued that early injury can impair the future ability of the brain to acquire new capabilities. This is difficult to assess empirically due to the presence of many age-dependent confounders.

Video gaming is a popular pastime for young males, having been shown to have both positive and negative effects on players' mental health. The aim of the current study was to ascertain the rate that male video gamers may seek mental health support for mental ill-health generally. The secondary aims were to (a) identify the most prolific barriers to seeking help; (b) determine how confident this group is to seek help; and (c) whether the variance in help-seeking efficacy be explained, in part, by age, hours gamed, self-esteem, and social capital.
