The Uprise of Human Leishmaniasis in Tuscany, Central Italy: Clinical and Epidemiological Data from a Multicenter Study.

Human leishmaniasis is facing important epidemiological changes in Southern Europe, driven by increased urbanization, climate changes, emerging of new animal reservoirs, shifts in human behavior and a growing population of immunocompromised and elderly individuals. In this evolving epidemiological landscape, we analyzed the clinical and epidemiological characteristics of human leishmaniasis in the Tuscany region of Central Italy. Through a multicentric retrospective analysis, we collected clinical and demographic data about all cases of leishmaniasis recorded between 2018 and 2023.

Toscana virus (TOSV) meningitis with atypical characteristics: Report of two cases.

Toscana virus (TOSV) is an emerging cause of central nervous system (CNS) infections, especially in endemic countries during summer. Cerebrospinal fluid (CSF) is usually clear, with < 500 leukocytes/mm, normal glucose (> 60 % serum glucose) and normal (< 45 mg/dL) to slightly increased protein levels. Here we present two cases of TOSV meningitis with misleading CSF characteristics observed at Santa Maria Annunziata Hospital (Bagno a Ripoli, Florence, Italy).

When it rains, it pours: Early treatment with tecovirimat of cardiac complications associated with monkeypox infection in a person with HIV and previously undiagnosed Lyme disease. A case report.

Cardiac involvement, such as myocarditis and pericarditis, can be a severe complication of monkeypox virus (mpox) infection and could be related to other co-infections with cardiac involvement. Tecovirimat is an antiviral specifically designed to inhibit smallpox infection diffusion and approved by the FDA for other Orthopoxvirus infections; its efficacy in mpox-infected patients is not well established. We present the case of a cardiac complication during mpox infection in a previously undiagnosed Lyme disease in a 42-year-old man living with HIV.

Nonlinear Dynamic Response of Nanocomposite Microbeams Array for Multiple Mass Sensing.

A nonlinear MEMS multimass sensor is numerically investigated, designed as a single input-single output (SISO) system consisting of an array of nonlinear microcantilevers clamped to a shuttle mass which, in turn, is constrained by a linear spring and a dashpot. The microcantilevers are made of a nanostructured material, a polymeric hosting matrix reinforced by aligned carbon nanotubes (CNT). The linear as well as the nonlinear detection capabilities of the device are explored by computing the shifts of the frequency response peaks caused by the mass deposition onto one or more microcantilever tips.

Unusual nonlinear switching in branched carbon nanotube nanocomposites.

In this experimental study, we investigate the nonlinear dynamic response of nanocomposite beams composed of polybutylene terephthalate (PBT) and branched carbon nanotubes (bCNTs). By varying the weight fraction of bCNTs, we obtain frequency response curves for cantilever specimens under harmonic base excitations, measuring the tip displacement via 3D scanning laser vibrometry. Our findings reveal a surprising nonlinear softening trend in the steady-state response of the cantilevers, which gets switched into hardening for higher bCNT weight fractions and increasing oscillation amplitudes.

Safety and Efficacy of Outpatient Treatments for COVID-19: Real-Life Data from a Regionwide Cohort of High-Risk Patients in Tuscany, Italy (the FEDERATE Cohort).

Early COVID-19 treatments can prevent progression to severe disease. However, real-life data are still limited, and studies are warranted to monitor the efficacy and tolerability of these drugs. We retrospectively enrolled outpatients receiving early treatment for COVID-19 in 11 infectious diseases units in the Tuscany region of Italy between 1 January and 31 March 2022, when Omicron sublineages BA.

Three-part humeral head fractures treated with a definite construct of blocked threaded wires: finite element and parametric optimization analysis.

Background: Mini open reduction and percutaneous fixation of three-part humeral head fracture with blocked threaded wires has demonstrated functional results similar to locking plates or intramedullary nails but with significantly lower major complication rate. In the context of three-part humeral head fractures, we performed a parametric optimization through a finite element analysis of a recently published construct to verify if the encouraging clinical results can be supported by a more rigorous investigation from a mechanical viewpoint.

Materials And Methods: The 2-dimensional geometry of a three-part proximal humerus fracture synthetized with a system of blocked threaded wires was created.

Real-Life Experience with Pomalidomide plus Low-Dose Dexamethasone in Patients with Relapsed and Refractory Multiple Myeloma: A Retrospective and Prospective Study.

The treatment of Myeloma after the second relapse is still challenging. The aim of the study was to investigate the outcomes of the POM-DEX regimen in real clinical practice. We retrospectively and prospectively analyzed 121 patients with MM treated with POM-DEX in three Italian sites in Tuscany.

The Role of PET in Supratentorial and Infratentorial Pediatric Brain Tumors.

Objective: This review aims to provide a summary of the clinical indications and limitations of PET imaging with different radiotracers, including 18F-fluorodeoxyglucose (18F-FDG) and other radiopharmaceuticals, in pediatric neuro-oncology, discussing both supratentorial and infratentorial tumors, based on recent literature (from 2010 to present).

Methods: A literature search of the PubMed/MEDLINE database was carried out searching for articles on the use of PET in pediatric brain tumors. The search was updated until December 2020 and limited to original studies published in English after 1 January 2010.

Optimal Design of CNT-Nanocomposite Nonlinear Shells.

Carbon nanotube/polymer nanocomposite plate- and shell-like structures will be the next generation lightweight structures in advanced applications due to the superior multifunctional properties combined with lightness. Here material optimization of carbon nanotube/polymer nanocomposite beams and shells is tackled via ad hoc nonlinear finite element schemes so as to control the loss of stability and overall nonlinear response. Three types of optimizations are considered: variable through-the-thickness volume fraction of random carbon nanotubes (CNTs) distributions, variable volume fraction of randomly oriented CNTs within the mid-surface, aligned CNTs with variable orientation with respect to the mid-surface.

Understanding COVID-19 nonlinear multi-scale dynamic spreading in Italy.

The outbreak of COVID-19 in Italy took place in Lombardia, a densely populated and highly industrialized northern region, and spread across the northern and central part of Italy according to quite different temporal and spatial patterns. In this work, a multi-scale territorial analysis of the pandemic is carried out using various models and data-driven approaches. Specifically, a logistic regression is employed to capture the evolution of the total positive cases in each region and throughout Italy, and an enhanced version of a SIR-type model is tuned to fit the different territorial epidemic dynamics via a differential evolution algorithm.

In an era where the cost of care in oncology is rising, suggestions of new frameworks that may help in orienting biomarker discovery are highly desirable. We propose a different perspective for looking at survival data, which we call 'death pace' analysis, which focuses on the variation of the gap between survival curves over time and that may make it easier to identify subpopulations with distinct predictive molecular features. Introduction: Helicobacter pylori is one of the most common causes of human gastritis. Recently, a new agent has been isolated, which also causes a gastritis. It has been initially named Gastrospirillum hominis and renamed Helicobacter heilmannii (Hh).

In the last few years the importance of GBS as the cause of serious neonatal sepsis has become more evident. The number of cases of this infection clearly exceeds the number of other congenital infections, for which antenatal screening is performed. Asymptomatic colonization of the genital tract of the pregnant woman has the most important role in transmission of GBS and several risk factors are connected to neonatal infection.

Microevolution in Perugia: isonymy 1890-1990.

The distribution of surnames in the population of the Comune of Perugia, as it existed in the memory banks of the Municipality Computer in autumn 1991, was studied by age and place of birth. Fisher's alpha and Karlin-McGregor's v were estimated in the total population, in persons born before 1901, and in persons born in the nine decades thereafter, ending with the period 1981-1991, for immigrants and for natives of Perugia, respectively. The wealth of surnames was significantly higher in immigrants than in natives of Perugia, as detected by alpha, v and by the log-log regression of the corresponding distributions.

[Splenic embolization and hepatic chemoembolization: combined transcatheter treatment of hepatocellular carcinoma in cirrhosis with hypersplenism].

Liver cirrhosis with hypersplenism is often associated with HCC. In many such cases, chemoembolization (TACE) may be very difficult because of the high incidence of hemorrhagic complications due to treatment and/or following portal hypertension, as well as for poor hematologic tolerance to antiblastic drugs in cirrhotic patients. Six patients with nodular HCC and cirrhosis (Child B) with hypersplenism were treated by combined TACE and partial splenic embolization (PSE) to reduce splenic size and to improve hematologic and hepatic function rates.

The distribution of surnames in the population of the town of Ferrara, as it existed in the memory banks of the Municipality Computer in June 1990, was studied by sex, age and place of birth of residents. Random isonymy was studied separately in persons born before 1901, and in persons born in the nine decades thereafter, ending with the period 1981-90. Isonymy was higher in the older age groups studied.

Isonymy in records of births and deaths in Ferrara.

Surname distributions were studied in records of male and female births in Ferrara in the period 1982-89, and in records of male and female deaths in the same period. Average year of birth and standard deviation was 1985 +/- 2.3 for the birth series, and 1912 +/- 14.

Linkage of biopsy, cancer, and population records aimed at the estimation of family risks in neoplasia: a pilot study.

Study Objective: The aim was to link individual demographic and medical records into sibships to obtain the sibling distribution of biopsies and cancers, and thereby calculate heritability and recurrence risks in families, thus aiding early diagnosis and prevention of cancers.

Design: The 157,823 individual records of the inhabitants of the town of Ferrara in Italy were automatically linked into 106,821 sibships. A 10% sample (10,842 sibships) was then extracted from the distribution of sibships and tabulated, for linkage to medical records.

Isonymy in emigrants from Ferrara in 1981-1988.

The distribution of surnames in the emigrants from the population of the town of Ferrara in the period 1981-88 was studied by sex and place of birth, namely Ferrara versus other places. Emigrants born in Ferrara were defined as first time emigrants and those who had previously immigrated to Ferrara were defined second time emigrants. It was found that random isonymy is smaller in second time emigrants.

[Clinical evaluation of the use of somatostatin (VAL 787) in the treatment of upper digestive hemorrhage].

Personal experience with 30 patients treated with Somatostatin for upper gastrointestinal bleeding is reported. The conclusion is drawn that this drug, reducing chloro-peptic secretion and hepatic portal blood flow, can reduce and control bleeding and postpone the surgical operation, which has a high rate of mortality.

Isonymy and migration distance.

The distribution of surnames of the living immigrants in the population of the town of Ferrara was studied by sex of immigrants and by distance of their place of origin from Ferrara. It was found that isonymy decreases with distance from the place of origin and that there is sexual dimorphism in migration distance: females migrate preferentially at short distances and males at long and very long distances. As indicators of the abundance of surnames in a distribution, the common ecological indices derived from entropy were used, and compared between migration ranges.
