We report the case of a 45-year-old male patient, a smoker, with a known condition of situs inversus totalis (SIT), who was diagnosed with an alveolar consolidation process during a chest imaging examination. Thorough medical investigations, including a surgical lung biopsy, resulted in the diagnosis of pulmonary infarction. The patient's clinical picture began suddenly, with chest pain of a stabbing character on the left side, inspiratory dyspnea, one episode of hemoptysis, fever (40°C), chills, and profuse sweating.

Obstructive sleep apnea syndrome (OSAS) is a very frequent and underdiagnosed pathology with important social and economic implications. Issues related to passive smoking have recently been addressed in the specialty literature, and they are particularly relevant from two perspectives: the effects on health and implications in the initiation of smoking among youth. The mechanisms by which tobacco smoke is involved in the pathology of heart disease are numerous, and are based not on a single action, but involve multiple factors. The effects of "second-hand" smoking on the cardiovascular system are evident in cardiovascular disease biomarkers seen in nonsmokers and the increased risk of mortality.

Aim of this paper is to compare the new polysomnography staging manual according to American Academy of Sleep Medicine with Rechtschaffen & Kales rules, underlying the newly introduced aspects and illustrated with polysomnographic images. The new rules simplify the staging but increase the time of electrodes application.
