We report the case of a young adult male who presented to our ICU on two separate occasions with hypoxemic respiratory failure requiring intubation. Esophageal manometry (EM) guided positive end expiratory pressure (PEEP) titration was utilized during both ICU admissions to improve oxygenation and aid in extubation with spontaneous breathing trials performed on higher-than-normal PEEP settings and successful liberation on both occasions.

We aim to compare the valve size, area, and transaortic mean gradients in SAVR patients before and after the implementation of TAVR since being approved by the Food and Drug Administration in 2011.
Methods: Patients who underwent a bioprosthetic SAVR placement were divided into two groups based on the date of procedure: the early pre-TAVR implementation group (years 2011-2012) and the contemporary post-TAVR group (years 2019-2020).

View Article and Find Full Text PDFThe present work explores the effect of a stress relieving heat treatment on the microstructure, tensile properties and residual stresses of the laser powder bed fused AlSi9Cu3 alloy. In fact, the rapid cooling rates together with subsequent heating/cooling cycles occurred during layer by layer additive manufacturing production make low temperature heat treatments desirable for promoting stress relaxation as well as limited grain growth: this combination can offer the opportunity of obtaining the best compromise between high strength, good elongation to failure and limited residual stresses. The microstructural features were analysed, revealing that the high cooling rate, induced by the process, caused a large supersaturation of the aluminum matrix and the refinement of the eutectic structure.

While significant research has addressed many of the effects of acute pain episodes and end-organ damage connected to this disease, little has approached the chronic pain state associated with this condition.
Recent Findings: Associated chronic pain represents a significant detractor from the quality of life experienced by these patients, affecting over half of those with SCD on more days than not.
Purpose Of Review: Multiple sclerosis (MS) is an autoimmune disorder of the central nervous system that can lead to severe physical, cognitive, and neurological deficits that often manifest in young adults. Central neuropathic pain is a common presenting symptom, often prompting patients to seek treatment with opioids, NSAIDS, antiepileptics, and antidepressants despite minimal effectiveness and alarming side-effect profiles. Additionally, spasticity occurs in more than 80% of MS patients and is an important consideration for further study in treatment.

Purpose Of Review: The purpose of the following review is to summarize the most recent understanding of migraine pathophysiology, as well as of basic and clinical science pharmacologic literature regarding the development of calcitonin gene receptor peptide (CGRP) antagonists as a novel therapeutic modality for the treatment of migraine headaches. A review is provided of erenumab, the first of its class FDA approved CGRP antagonist.
Recent Findings: Despite its high prevalence, the occurrence and treatment of migraine headaches is poorly understood.
