We describe a 48-year-old woman with loin pain-hematuria syndrome. Her renal abnormalities included conspicuous microaneurysmal and glomeruloid (plexiform) angiomatous changes. The deposition of both properdin and the C5b-9 complex, as well as the usual C3, in arterioles argues for complement activation.

Prostatic tissue specimens derived from transurethral resections of patients with metastatic prostate cancer were analyzed for genetic alterations in the hormone-binding domain of the androgen receptor (AR) gene. Direct sequencing of the polymerase chain reaction-derived DNAs of 6 of 24 specimens revealed a codon 877 mutation (ACT-->GCT, Thr-->Ala) in the hormone-binding domain of the AR gene. This same AR mutation has been reported previously in a metastatic prostate cancer cell line, LNCaP, where this mutation confers upon the AR an altered ligand-binding specificity which is stimulated by estrogens, progestagens, and antiandrogens.

In feeding experiments with piglets of the country species variously treated straw materials were used in concentrate--straw meal mixtures after the traditional weaning date (49th day of life) and studies were made of the fermentation of volatile fatty acids (VFA) in the large intestine (colon) and of the concentration of selected parameters of the protein, fat, carbohydrate and mineral metabolism in the blood serum of the animals. The concentrate--straw mixtures with a 10% straw quota contained untreated (II), HCl treated (HCl treatment without steaming, without (III) and with neutralization, IV) as well as partly hydrolysed straw meal (HCl treatment with subsequent steaming, without (V) and with neutralization with CaCO3, VI) were tested in comparison with sole feeding of concentrate (I) and its combination with wheat bran (VII). With regard to the fermentation processes in the large intestine in a comparison of the feeding groups (I-VII) the highest concentration and production quota of VFA was registered in I and the lowest in II.

The ad libitum influence of various quotas of partly hydrolysed straw meal (PHSM) on the fermentation and production of volatile fatty acids (VFA) in the colon in the measuring periods 2nd and 8th weeks of keeping was checked within a feeding experiment with weaned piglets of the country species (42nd-98th day of life; 1st-8th week of keeping). The straw hydrolysis product was fed in a parallel experiment without or after neutralization with CaCO3 in quotas of, 5, 10 and 15% related to dry matter in comparison to the feeding of sole concentrate feed. The measuring in the 2nd week of keeping (adaptation phase) was blurred by the diarrhea in the groups with regard to the concentration, production and molar proportion of VFA.

In studies with weaned piglets of the land race the usability of partly hydrolysed straw meal (PSM) in concentrate - straw meal mixtures was tested over a period of 56 days (42nd-98th day of life; 1st-8th week of feeding. The hydrolysed straw product was there tested in a parallel experiment each without and after neutralization (by means of CaCO3) in quotas of 5, 10 and 15% as supplement to the concentrate feed (based on dry matter) in comparison to a sole concentrate feeding. Daily weight gain and feed and energy expenditure as well as nutrient digestibility and the buffer effect of the PSM rations were ascertained.

Arctic-breeding shorebirds collected in western Washington state during winter and spring, and a comparative sample collected in coastal California during the winter were analyzed for organochlorine contaminants to determine the potential impact of these residues on populations of peregrine falcons (Falco peregrinus) and merlins (F. columbarius) which prey upon shorebirds in western Washington. Dunlins (Calidris alpina), an important winter prey for falcons in western Washington, were collected between 1975 and 1981.

Contrast nephropathy.

Am Fam Physician

June 1987

Preexisting renal insufficiency, diabetes mellitus and advanced age are risk factors for the development of contrast nephropathy after radiographic procedures in which hypertonic contrast solutions are infused. Preventive measures include careful patient selection, avoidance of volume depletion and, whenever possible, allowance of recovery time between radiographic procedures. The newer contrast agents appear to be safer than the older agents, and they provide improved radiographic detail.

A significant question relative to treatment and disappearance of symptoms is raising the bite in combination with a maxillary bite plane splint in patients with TMJ muscle dysfunction. The objective of this research was to investigate the effect, if any, occlusal bite plane splints have on the latency and duration of the mention tap silent period and particularly whether the degree to which the bite is raised is of significance. Recordings were made in healthy young adults from both anterior temporal and masseteric muscles while subjects were clenching at their 70% maximum clenching effort.
