Purpose: The randomized GeparOla trial reported comparable pathological complete response (pCR) rates with neoadjuvant containing olaparib vs. carboplatin treatment. Here, we evaluate the association between functional homologous repair deficiency (HRD) by RAD51 foci and pCR, and the potential of improving patient selection by combining RAD51 and stromal tumor infiltrating lymphocytes (sTILs).

Background: The GENEVIEVE study, comparing neoadjuvant cabazitaxel versus paclitaxel in triple-negative breast cancer (TNBC) and luminal B/human epidermal growth factor receptor 2 (HER2)-negative breast cancer (BC), previously reported significant differences in pathological complete response (pCR) rates. Effects on long-term outcome are unknown.

Patients And Methods: GENEVIEVE randomized patients with cT2-3, any cN or cT1, cN+/pN+, centrally confirmed TNBC or luminal B/HER2-negative BC (latter defined as estrogen/progesterone receptor-positive and >14% Ki-67-stained cells) to receive either cabazitaxel 25 mg/m q3w for four cycles or paclitaxel 80 mg/m weekly for 12 weeks.

Background: A structured risk assessment of patients with validated and evidence-based tools can help to identify modifiable factors before major surgeries. The Protego Maxima trial investigated the value of a new digitized risk assessment tool that combines tools which can be easily used and implemented in the clinical workflow by doctors and qualified medical staff. The hypothesis was that the structured assessment and risk-grouping is predictive of short-term surgical quality reflected by complications and overall survival.

Objective: Previous research on problem-based learning (PBL) describes that videotaped observations develop meaningful insights into cognitive processes in tutorial groups. Analysis regarding the amount of prior knowledge on learning achievement has not been investigated in medical education so far, although both are key factors of PBL success. Thus, we intended to analyse videos of digital problem-based learning (dPBL) sessions, focusing on knowledge acquisition and interaction dynamics among groups with different levels of prior knowledge to reveal any distinctions.

Article Synopsis
  • Malignancies can lead to severe bile duct narrowing, increasing the risk for bacterial cholangitis, which is often treated with antibiotics following drainage procedures.
  • A study analyzed the effectiveness of short antibiotic therapy (≤6 days) versus long therapy (≥7 days) in patients with biliary strictures and cholangitis, finding no increased risk of recurrent cholangitis with short therapy.
  • The study recommends monitoring for multidrug-resistant organisms, and suggests that using metal stents can also reduce the risk of recurring cholangitis in these patients.

  • A phase II study assessed the effectiveness of ribociclib combined with maintenance endocrine therapy (ET) in women with hormone receptor-positive and HER2-negative metastatic breast cancer after initial chemotherapy.
  • The study involved 43 patients taking ribociclib + ET and 10 patients on ET alone, showing that the combination therapy resulted in a significantly longer median progression-free survival (12.4 months) compared to ET alone (4.75 months).
  • Although some patients experienced serious adverse effects, the overall findings suggest that ribociclib plus ET could be a promising treatment option for maintaining disease stability in this patient population.

Article Synopsis
  • Recent studies show that ongoing HCV infections are mainly transmitted through drug use and sexual behaviors, particularly among men who have sex with men (MSM), but the details of transmission and infection progression remain unclear.
  • After following 161 patients with recently acquired HCV infections over 10 years, it was found that most were male MSM, many of whom were also HIV-positive, and their main transmission risks were sexual behavior and drug use.
  • The findings indicate low spontaneous clearance rates of the infection, significant phylogenetic clustering among MSM cases, and a rise in specific HCV genotypes over time, highlighting the need for targeted interventions in high-risk groups.

Article Synopsis
  • The study aimed to evaluate olfactory dysfunction (OD) in both SARS-CoV-2 positive and negative patients to see if it could help identify COVID-19 infections early.
  • Results showed that OD was present in 65.6% of COVID-19 positive patients and was more common in males and those with certain health conditions, with a notable increase in cases linked to the Delta variant compared to Omicron.
  • Despite these findings, the sensitivity (69%) and specificity (64%) of OD as a diagnostic tool for identifying COVID-19 were deemed insufficient for it to be used routinely in clinical settings.

Introduction: Major surgery is associated with a high risk for postoperative complications, leading to an increase in mortality and morbidity, particularly in frail patients with a reduced cardiopulmonary reserve. Prehabilitation, including aerobic exercise training, aims to improve patients' physical fitness before major surgery and reduce postoperative complications, length of hospital stay and costs. The purpose of the study is to assess the usability, validity and safety of an app-based endurance exercise software in accordance with the Medical Device Regulation using wrist-worn wearables to measure heart rate (HR) and distance.

Synchronous bilateral breast cancer (sBBC) occurs after both breasts have been affected by the same germline genetics and environmental exposures. Little evidence exists regarding immune infiltration and response to treatment in sBBCs. Here we show that the impact of the subtype of breast cancer on levels of tumor infiltrating lymphocytes (TILs, n = 277) and on pathologic complete response (pCR) rates (n = 140) differed according to the concordant or discordant subtype of breast cancer of the contralateral tumor: luminal breast tumors with a discordant contralateral tumor had higher TIL levels and higher pCR rates than those with a concordant contralateral tumor.

Background: Endoscopic retrograde cholangiopancreatography (ERCP) is the mainstay for interventional treatment of biliopancreatic diseases, and guidewires are decisive for successful treatment.

Methods: A randomized, multicenter, open-label study was conducted in patients in whom a guidewire was required in ERCP. Success rate and duration of the investigation were evaluated by randomly applying hydrophilic guidewires with a straight tip (ST) or an angled tip (AT) configuration, with normal (type NF) or enhanced (type EF) flexibility.

Article Synopsis
  • The study analyzed real-world data from 145 patients with stage IV somatostatin-neuroendocrine tumors (siNET) to compare the survival outcomes based on different treatment regimens.
  • Survival rates showed that all patients had a high one-year overall survival (OS) rate of 93.8%, but treatment regimens like chemotherapy and multimodal approaches did not significantly enhance survival compared to somatostatin analogues (SSA) alone.
  • The resection of the primary tumor greatly improved the five-year survival rates, indicating its crucial role in patient outcomes.

Effective antibiotic therapy of cerebral infections such as meningitis or ventriculitis is hindered by low penetration into the cerebrospinal fluid (CSF). Because continuous infusion of meropenem and vancomycin and routine therapeutic drug monitoring (TDM) have been proposed to optimize antimicrobial exposure in ventriculitis patients, an individualized dosing strategy was implemented in our department. We present a retrospective analysis of meropenem and vancomycin concentrations in serum and CSF in the first nine ventriculitis patients treated with continuous infusion and TDM-guided dose optimization aiming at 20-30 mg/L.
