Article Synopsis
  • Trauma, particularly traumatic brain injury (TBI), is a significant cause of maternal mortality during pregnancy, yet there is a lack of guidelines for managing these cases.
  • An international survey with 122 physicians from 110 centers revealed that many do not have specific protocols for TBI in pregnant patients and highlighted a preference for using MRI and varying practices regarding transfusions and treatment methods.
  • The findings indicate substantial inconsistency in how severe TBI is managed in pregnant individuals, suggesting a need for more research and education to standardize care.

Article Synopsis
  • Pediatric patients with congenital heart disease (CHD) often receive low dose ionizing radiation during cardiac catheterizations (CC) for diagnosis and treatment, but the long-term cancer risks associated with this radiation are not well understood.
  • A study was conducted with a French cohort of over 17,000 children who had their first CC between 2000 and 2013, tracking them for factors leading to lympho-hematopoietic malignancies.
  • The results showed no increased risk of these cancers related to the low dose radiation from CC, however, more extensive studies are suggested for better understanding the relationship between radiation exposure and cancer risk.

Background: Ventricular arrhythmias and sudden death are recognized complications in tetralogy of Fallot. Electrophysiological studies (EPS) before pulmonary valve replacement (PVR), the most common reintervention in tetralogy of Fallot, could potentially inform therapy to improve arrhythmic outcomes.

Methods: A prospective multicenter study was conducted to systematically assess EPS with programmed ventricular stimulation in patients with tetralogy of Fallot referred for PVR from January 2020 to December 2021.

Moderate and severe traumatic brain injury (TBI) are major causes of disability and death. In addition, when TBI occurs during pregnancy, it can lead to miscarriage, premature birth, and maternal/fetal death, engendering clinical and ethical issues. Several recommendations have been proposed for the management of TBI patients; however, none of these have been specifically applied to pregnant women, which often have been excluded from major trials.

The vertebrate left-right axis is specified during embryogenesis by a transient organ: the left-right organizer (LRO). Species including fish, amphibians, rodents and humans deploy motile cilia in the LRO to break bilateral symmetry, while reptiles, birds, even-toed mammals and cetaceans are believed to have LROs without motile cilia. We searched for genes whose loss during vertebrate evolution follows this pattern and identified five genes encoding extracellular proteins, including a putative protease with hitherto unknown functions that we named ciliated left-right organizer metallopeptide (CIROP).

Article Synopsis
  • Bisphenols and phthalates impact androgen receptor signaling, which influences KLK3 secretion, suggesting environmental factors may affect this process.
  • A study measured serum KLK3 levels in healthy women from high and low environmental impact areas, finding higher levels in the high impact group, especially during ovulation.
  • Significant differences in KLK3 levels corresponded with lower progesterone levels in the high impact group during the luteal phase, indicating a potential link between environmental pollution and female reproductive health.

Purpose: The COCCINELLE study is a nationwide retrospective French cohort set up to evaluate the risk of cancer in patients who undergone cardiac catheterisation (CC) procedures for diagnosis or treatment of congenital heart disease during childhood.

Participants: Children who undergone CC procedures from 1 January 2000 to 31 December 2013, before the age of 16 in one of the 15 paediatric cardiology departments which perform paediatric CC in mainland France were included. The follow-up started at the date of the first recorded CC procedure until the exit date, that is, the date of death, the date of first cancer diagnosis, the date of the 18th birthday or the 31 December 2015, whichever occurred first.

Article Synopsis
  • Advances in pediatric cardiology have enhanced survival rates for children with inherited cardiac disorders, but there's limited research on how these conditions affect their quality of life (QoL) and physical activity.
  • The QUALIMYORYTHM trial is a multicenter study aimed at comparing the QoL of children aged 6 to 17 with inherited cardiac arrhythmias or cardiomyopathies to that of healthy peers and examining various factors influencing their QoL.
  • Results from this trial seek to provide stronger evidence for guidelines regarding sports participation for young athletes with inherited cardiac disorders, moving beyond reliance on expert opinions.

Article Synopsis
  • - The study focuses on the management of anomalous aortic origin of a coronary artery, which can be linked to sudden death in young individuals, especially those with an inter-arterial course.
  • - Over a 25-year period, 100 patients were diagnosed, with higher rates of syncope and sudden death among those with an inter-arterial course compared to others (23.4% vs 0%).
  • - Surgical repair was performed on 61 patients, all of whom had the inter-arterial course and were asymptomatic after an average follow-up of about 5 years, suggesting that surgical intervention is a viable option even for asymptomatic cases.

Article Synopsis
  • The study evaluated the effect of pulmonary valve replacement (PVR) on the incidence of ventricular arrhythmias in tetralogy of Fallot (TOF) patients who have implantable cardioverter-defibrillators (ICDs).
  • Results showed that among the 165 TOF patients monitored, those who underwent PVR experienced significantly fewer appropriate ICD therapies after the surgery compared to before, indicating a reduction in arrhythmia burden.
  • The findings suggest that PVR may play a crucial role in decreasing the risk of arrhythmias and highlight the importance of considering PVR in the treatment plans for high-risk TOF patients.

About 10-20% of patients with Kawasaki disease (KD) are unresponsive to intravenous immunoglobulin (IVIg) and are at increased risk of coronary artery abnormalities (CAAs). Early identification is critical to initiate aggressive therapies, but available scoring systems lack sensitivity in non-Japanese populations. We investigated the accuracy of 3 Japanese scoring systems and studied factors associated with IVIg unresponsiveness in a large multiethnic French population of children with KD to build a new scoring system.
