Background: Neurosurgical training requires a deep understanding of brain anatomy, especially white matter fiber pathways, to enhance surgical precision. Traditional dissection techniques, such as Klingler's white matter dissection, are essential, but newer methods can provide additional clarity. This study explores the application of a fluorescent-assisted technique to improve the visualization and understanding of white matter fibers during neurosurgical training.

The transmeatal extension of the retrosigmoid approach is an important procedure used in the treatment of various pathologies affecting the posterior fossa, petroclival region, and jugular foramen. Mastering this technique requires a high level of manual skill, particularly in temporal bone drilling. The objective of this study was to describe an easily accessible and cost-effective model of the transmeatal extension of the retrosigmoid approach using cadaveric sheep heads.

Objective: Chronic inflammation's role in the pathogenesis, development, and rupture of vascular malformations is undebated. Advanced magnetic resonance imaging techniques with vessel wall studies, specifically Black Blood (bbMRI), may offer insights into vascular wall instability and predict rupture. This case series aims to assess bbMRI as a predictive diagnostic tool for brain arteriovenous malformations (bAVMs) rupture, suggesting early treatment.

Midbrain cavernous malformations (MCMs) are rare and dangerous taken the important structures and tracts located in this segment of the brainstem. MCM treatment is still controversial, and surgical resection is basically indicated in cases of recurrent hemorrhage and progressive neurologic deterioration. The optimal moment to operate ruptured MCM is in the subacute stage.

Spontaneous intracerebral hemorrhage (ICH) represents a critical and potentially devastating medical event resulting from the rupture of intracerebral vessels. Patients afflicted with ICH face an increased risk of venous thromboembolism (VTE) due to factors such as immobility. However, determining the ideal timing for initiating venous thromboembolism thromboprophylaxis (TP) remains uncertain, as it may carry the potential risk of exacerbating hematoma expansion.

  • Arteriovenous malformations (AVMs) are complex congenital lesions requiring advanced surgical skills, and human placentas offer a realistic training model for AVM surgery.
  • The study involved creating AVM-type lesions in 15 human placentas using synthetic materials, allowing neurosurgeons to practice microsurgical techniques in a controlled environment.
  • Results showed that this model effectively simulates the key characteristics of AVMs, making it a valuable tool for training in vascular microsurgery.

Background And Objective: The idea of watertight dural closure (WTDC) seems extremely obvious to avoid complications such as cerebrospinal fluid (CSF) leak and infection, especially in spinal and posterior fossa surgeries. Nonetheless, several studies have shown that leaving the dura open is not associated with an increased risk of complications. The aim of this systematic review and meta-analysis is to compare non-WTDC and WTDC in patients undergoing supratentorial craniotomy regarding the risk of postoperative complications.

Cavernous angioma is a sinusoidal dilatation covered by a single layer of endothelium, separated by a collagen matrix with elastin and smooth muscle.1 The prevalence in the general population is estimated at 0.4% to 0.

Unlabelled: Central nervous system (CNS) malignant neoplasms may lead to venous thromboembolism (VTE) and bleeding, which result in rehospitalization, morbidity and mortality. We aimed to assess the incidence of VTE and bleeding in this population.

Methods: This systematic review and meta-analysis (PROSPERO CRD42023423949) were based on a standardized search of PubMed, Virtual Health Library and Cochrane (n = 1653) in July 2023.

Cavernous malformations (CMs) are rare, often oligosymptomatic vascular lesions. Common manifestations include seizures and focal neurological deficits. Depending on the symptoms, location, size, and risk factors of bleeding, such as the presence of a developmental venous anomaly, the injury can be highly morbid.

New hippocampal neurons are continuously generated in the adult human brain. Several studies have demonstrated that the proliferation of hippocampal cells is strongly influenced by a variety of stimuli, including pesticides exposure. These effects are particularly important because neurogenesis dysregulation could be associated with the decline of neuronal and cognitive functions and the possible development of neuropsychiatric disorders.

Introduction: Deep cavernomas of eloquent areas, located in the region of the basal nuclei and thalamus, account for 9 to 36% of these encephalic vascular malformations. Internal capsule cavernomas are particularly challenging, as they are surrounded by important projection fibers and their manipulation can lead to permanent deficits. To demonstrate through surgical cases that cavernomas of the internal capsule can be approached by frontal craniotomy, via the superior frontal sulcus, in a curative manner and with low morbidity.

  • Mutations in the PTEN gene are linked to PTEN hamartoma tumor syndrome, which increases the risk of tumors and vascular issues, manifesting in conditions like Cowden syndrome and more.
  • A case study of a 27-year-old woman showed persistent visual symptoms and headaches, eventually revealing a complex issue with multiple recurrent dural arteriovenous fistulas (dAVFs) despite initial treatments.
  • Genetic testing identified a PTEN variant associated with Cowden syndrome, marking the emergence of a novel PTEN-related phenotype due to the patient's unique and aggressive vascular complications, despite the absence of typical tumor presentations.

Article Synopsis
  • - Intraventricular hemorrhage (IVH) is a severe condition with high mortality, and recent studies suggest that neuroendoscopic treatment, specifically endoscopic brainwashing (EBW), may improve outcomes compared to traditional external ventricular drainage (EVD).
  • - A retrospective analysis of adult IVH patients showed that those receiving EBW had larger hemoventricles and more chronic conditions but exhibited lower short-term mortality and better long-term outcomes compared to EVD patients.
  • - The study concluded that EBW appears to lead to improved functional outcomes over time, with a greater percentage of patients achieving favorable results than those treated with EVD alone.

Background And Objective: Neurosurgery relies heavily on advanced manual skills, necessitating effective training models for skill development. While various models have been utilized, the human placenta has emerged as a promising candidate for microneurosurgical training due to its anatomical similarities with cerebral vasculature. However, existing placenta models have primarily focused on simulating superficial procedures, often neglecting the complexities encountered in deep operative fields during cranial surgeries.

Article Synopsis
  • * A total of 42 brainstems were analyzed, revealing a significant increase in surgical exposure from 42 mm² to 159.6 mm² after QLC resection, aiding in procedures involving the pons and cerebellum.
  • * After evaluating 59 patients who underwent this surgery, it was found that while some experienced temporary side effects like ataxia, all symptoms resolved within a year, and overall patient outcomes improved.
