Introduction: The purpose of this pilot study was to conduct periodic diabetes care visits in school, with the goal of promoting optimal management of diabetes for high-risk youth.

Method: A convenience sample of 27 students receiving care at a university-affiliated children's diabetes center and enrolled in a large urban school district received the diabetes visit intervention. Intervention effect was measured by parent survey of home/school diabetes management practices, the Self-Efficacy for Diabetes Tool, and a diabetes care-provider survey of subjects' usual care, management competence, and glycemic control.

This pilot study assessed the impact of diabetes on overall quality of life (QOL) of 23 adolescents (12-16 years old). The impact of diabetes on daily life, satisfaction with life, and common worries and frustrations were explored through the Diabetes Quality of Life for Youth scale (DQOLY) and semistructured interviews. The four major themes that emerged from the interviews were (a) restrictions, (b) differentness, (c) negative emotion, and (d) adaptation.

Results of the Diabetes Control and Complications Trial announced in June 1993 provide convincing evidence that achievement of near-normal glycemic levels in insulin-dependent diabetic individuals decreases the microvascular complications of diabetes. School nurses should expect some changes in management of diabetes to emerge in the near future, with requirements for increased expertise and closer communication with diabetes care providers.

Recent data suggests that one of the major actions of sulfonylureas is to potentiate the anabolic cellular effects of insulin. This is the first study to examine the use of sulfonylureas as adjunctive therapy in newly-diagnosed type I diabetic children. A random, prospective, double blind study over 15 months, stratified by age at diagnosis, was conducted.
