Purpose: There are currently no specialized pediatric neurosurgeons in Pakistan. The extent to which this impacts the outcomes of children undergoing surgery for neurological conditions in the country is unclear. We aimed to investigate whether outcomes of brain tumor resections in children at our high-volume center in Pakistan were comparable to those performed by pediatric neurosurgeons in a large, validated US surgical database.

Being "between Scylla and Charybdis" is an idiom derived from Greek mythology to mean "between a rock and a hard place" and clinicians managing amyloid light-chain (AL) amyloidosis often find themselves in this predicament. AL amyloidosis is caused by monoclonal light chains, most commonly produced by CD-38 positive plasma cells in target organs. The disease usually involves significant cardiac and/or renal involvement, but the systemic nature of the disease often leads to variable and non-specific manifestations that can critically delay early diagnosis and treatment.

Prairie wetland ponds on the Great Plains of North America offer a diverse array of geochemical scenarios that can be informative about their impact on microbial communities. These ecosystems offer invaluable ecological services while experiencing significant stressors, primarily through drainage and climate change. In this first study systematically combining environmental conditions with microbial community composition to identify various niches in prairie wetland ponds, sediments had higher microbial abundance but lower phylogenetic diversity in ponds with lower concentrations of dissolved organic carbon ([DOC]; 10-18 mg/L) and sulfate ([SO ]; 37-58 mg/L) in water.

Background: The Surgical Infection Society (SIS) guidelines recommend against the use of surgical antibiotic prophylaxis (SAP) for low-risk patients undergoing elective laparoscopic cholecystectomies.

Methods: Using National Surgical Quality Improvement Program (NSQIP) data, 5440 pediatric patients were identified who underwent laparoscopic cholecystectomy from 2021 to 2022. Patients who had immunodeficiency, active malignancy, American Society of Anesthesiologists (ASA) physical status classification 3-5, procedure indicated for infection, emergent procedure, received intravenous antibiotics before the prophylaxis window, or missing SAP data were excluded.

Article Synopsis
  • Tube thoracostomy (TT) is used to drain fluid from the pleural cavity, but improper tube positioning occurs in 30% of cases, which can complicate treatment.* -
  • In a study involving 650 TT procedures on cadaver torsos, two techniques ("head" vs. "bed" direction) were compared to see which resulted in fewer instances of tube placement in lung fissures.* -
  • Results showed that tubes aimed "toward the bed" had a significantly lower placement rate in fissures (13%) compared to the "head" positioning (41%), suggesting that this technique could improve clinical outcomes.*

Article Synopsis
  • Esophageal squamous cell carcinoma (ESCC) shows high variability and its complex molecular and immune landscape complicates personalized treatment development.
  • Through genomic and AI-driven analysis of 120 Chinese ESCC cases, researchers identified four subtypes—differentiated, metabolic, immunogenic, and stemness—each with distinct features.
  • The stemness subtype has the worst prognosis and shows immune evasion traits, with high levels of intratumoural diversity, while specific markers like XCL1 influence both immune response and chemotherapy effectiveness.

Studies on chronic myeloid leukemia (CML) in the Gulf region are scarce, consisting of a survey and expert meeting that included 15 experts in 2023 which discussed CML diagnosis, testing, treatment objectives, toxicities, and discontinuation in the Gulf region. Most patients were reported to be in first-line therapy, and the most common treatments were imatinib/imatinib generic in first-line and dasatinib in second- and third-lines. Mutation analysis was not reported to be routinely performed at the time of diagnosis but rather in case of progression to accelerated/blast phase or any sign of loss of response.

Article Synopsis
  • The COVID-19 pandemic made it hard for candidates to do in-person interviews for fellowship programs, so they needed to find information online instead.
  • A group of recent graduates figured out what information is most important for choosing a program and how much of that info is available online.
  • They found that while many fellowship programs had basic info on the APSA website, important details like fellow case logs and research opportunities were often missing.
