Importance: The ability of surgeons to choose the right patient for fusion in addition to decompression when operating for degenerative spondylolisthesis with symptomatic spinal stenosis is debated. The addition of fusion increases risk, morbidity, and costs but has been claimed to give better results for selected patients.

Objective: To investigate whether following surgeons' opinions regarding fusion was associated with clinical outcomes.

Article Synopsis
  • The study aimed to evaluate the feasibility of using zoledronic acid (ZA) as a supplementary treatment for neovascular age-related macular degeneration (nAMD) in a randomized controlled trial.
  • A total of 40 nAMD patients were split into two groups, with one receiving ZA and the other a placebo, and the primary focus was on changes in visual acuity after treatment.
  • Results showed that the group receiving ZA had improved visual acuity compared to the placebo group, suggesting potential benefits that warrant further exploration, although the small sample size limits the strength of these findings.

Introduction: It is unclear whether moderate-to-vigorous physical activity (MVPA) is associated with a lower mortality risk, over and above its contribution to total physical activity volume.

Methods: 46,682 adults (mean age: 64 years) were included in a meta-analysis of nine prospective cohort studies. Each cohort generated tertiles of accelerometry-measured physical activity volume and volume-adjusted MVPA.

Article Synopsis
  • Cardiac allograft vasculopathy is a serious condition affecting heart transplant patients, characterized by increased thickness of coronary arteries, and despite using statins, it remains a top cause of death.
  • A clinical trial was conducted to investigate whether the cholesterol-lowering drug evolocumab could reduce this condition's severity in heart transplant recipients.
  • Though evolocumab significantly lowered LDL cholesterol levels in participants, it did not lead to a reduction in the thickness of coronary arteries after 12 months, and safety was not compromised during the study.

Introduction: Bacterial infection and Modic changes (MCs) as causes of low back pain (LBP) are debated. Results diverged between two randomised controlled trials examining the effect of amoxicillin with and without clavulanic acid versus placebo on patients with chronic LBP (cLBP) and MCs. Previous biopsy studies have been criticised with regard to methods, few patients and controls, and insufficient measures to minimise perioperative contamination.

  • The study aimed to see how jump load impacts knee issues in elite men's volleyball, using data from 65 players over three seasons.
  • Data included jump frequency and height from daily sessions and weekly knee complaint reports, analyzed using complex statistical models.
  • Findings showed no strong evidence linking weekly jump load to knee complaints or worsening symptoms, with slight differences in risk observed that suggest larger studies are needed for clearer conclusions.

Aims: Objective methods to determine statin adherence are requested to improve lipid management. We have recently established a method to detect reduced adherence to atorvastatin therapy with cut-off values based on the sum of atorvastatin and its major metabolites in the blood. We aimed to validate this method in patients with and without cardiovascular disease, and optimize previous cut-off values.

The relationship between recent (acute) training load relative to long-term (chronic) training load may be associated with sports injury risk. We explored the potential for modelling acute and chronic loads separately to address current statistical methodology limitations. We also determined whether there was any evidence of an interaction in the association between acute and chronic training loads and injury risk in football.

  • The DANBLOCK and BETAMI trials are assessing the long-term effectiveness and risks of beta-blocker therapy following a myocardial infarction (MI), particularly in patients with normal or slightly reduced left ventricular ejection fraction (LVEF).
  • Approximately 5700 patients are being randomized to receive either beta-blockers or no treatment, with the study aiming to track 950 primary endpoints, including recurrent MI and heart failure.
  • The results, expected by December 2024, could significantly influence clinical guidelines regarding beta-blocker use after MI for patients with normal or mildly reduced LVEF.

Objective: To investigate whether high-sensitivity cardiac troponin T (hsTnT) correlates to markers of disease activity in inflammatory arthritis (IA), and whether antirheumatic treatment influences hsTnT levels.

Methods: We assessed 115 patients with active IA (64 rheumatoid arthritis (RA), 31 psoriatic arthritis and 20 ankylosing spondylitis) before and after using methotrexate (MTX) alone or tumor necrosis factor inhibitor (TNFi) with or without MTX co-medication (TNFi±MTX). All patients starting with TNFi had been previously unsuccessfully treated with MTX monotherapy.

Mild or asymptomatic disease is now the dominating presentation of primary hyperparathyroidism (PHPT). However, bone involvement with decreased bone mineral density (BMD) and an increased risk of fractures has been demonstrated. Indications for parathyroidectomy (PTX) in mild PHPT have been debated for years.

Spondyloarthropathies (SpA) are associated with increased cardiovascular risk. Among possible mechanisms is the dysfunction of serum lipoproteins in regulating cell cholesterol homeostasis. Cholesterol efflux capacity (CEC)-the atheroprotective ability of HDL (high density lipoproteins) to accept cholesterol from macrophages-might predict cardiovascular disease independently of HDL-cholesterol levels.
