Background/objectives: HER2-positive breast cancer (HER2BC) is an aggressive subtype, with neoadjuvant treatment (NAT) aiming to achieve a pathological complete response (pCR) to improve long-term outcomes. Trastuzumab emtansine (T-DM1) has been established as the standard of care in the adjuvant setting for HER2BC patients who do not obtain pCR. The ATD study aimed to evaluate the real-world tolerability of T-DM1 in this setting.

Article Synopsis
  • The study evaluates the real-world effectiveness of the Pembrolizumab/Axitinib combination therapy for metastatic renal cell carcinoma (mRCC) in Italy, involving 170 patients treated between December 2020 and September 2023.
  • Most participants had clear-cell histology (83%), and the treatment showed a median progression-free survival (PFS) of 19.2 months, with 84.6% achieving disease control.
  • The study highlights key prognostic factors, including age and histology, that affect overall survival (OS) and establishes the combination's promise in clinical settings.

Article Synopsis
  • - Enfortumab vedotin (EV) is effective for patients with advanced urothelial carcinoma who have previously received platinum chemotherapy and immune therapy, despite lacking research on those treated with avelumab maintenance.
  • - A study of 182 patients showed a median overall survival of 12.7 months and a progression-free survival of 7.9 months, with 39% achieving a positive response to EV after avelumab treatment.
  • - The study confirms EV's effectiveness, suggesting it can be a viable option for patients previously treated with avelumab, with manageable side effects like grade ≥ 3 neuropathy and skin rash occurring in a minority of cases.

Check-Mate 274 has demonstrated the disease-free survival (DFS) benefit of adjuvant nivolumab in surgically treated muscle-invasive bladder cancer (MIBC). Since immunotherapy represents an expensive treatment with potential side effects, a better understanding of patient-specific risks of disease progression might be useful for clinicians when weighing the indication for adjuvant nivolumab. To identify the criteria for risk stratification of disease progression among MIBC patients eligible for adjuvant nivolumab.

Article Synopsis
  • Advances in immuno-oncology have improved treatment for renal cell carcinoma, but patients with "primary refractory" disease have poor outcomes; our study found a 19% prevalence of this group among 1,709 metastatic clear cell patients across 72 centers in 22 countries.
  • The highest primary refractory rate was 27% with nivolumab/ipilimumab, while pembrolizumab/lenvatinib had the lowest at 10%; those classified as primary refractory only had a median survival of 7.6 months compared to 55.7 months for non-primary refractory patients.
  • Significant predictors of survival for primary refractory patients included factors like prior nephrectomy and presence of bone/brain metastases, highlighting the complex

Article Synopsis
  • The study focuses on the effectiveness of pembrolizumab, an immune checkpoint inhibitor, for advanced urothelial carcinoma (UC) patients with poor performance status (ECOG-PS 2).
  • Among 1,040 patients, those with ECOG-PS 2 had significantly lower median overall survival (3.7 months) compared to those with ECOG-PS 0-1 (18.2 months), highlighting the challenges faced by this group.
  • Key factors affecting outcomes included the presence of liver metastases and disease progression during first-line therapy, indicating a need for careful evaluation of these patients' conditions.

Upper tract urothelial carcinoma (UTUC) accounts for the 5-10% of all urothelial carcinomas (UCs). In this analysis, we reported the real-world data from the ARON-2 study (NCT05290038) on the efficacy of pembrolizumab in patients with UTUC who recurred or progressed after platinum-based chemotherapy. Medical records of patients with metastatic UTUC treated with pembrolizumab as second-line therapy were reviewed from 34 institutions in 14 countries.

Article Synopsis
  • Atezolizumab was assessed for its safety in a diverse group of patients with pretreated urinary tract carcinoma (UTC) in the SAUL study, which included those typically excluded from clinical trials.
  • The study involved 1004 patients and found that 68 patients continued treatment for over 4 years, with 16% experiencing serious treatment-related adverse events.
  • Long-term results show a median overall survival of 8.6 months, with 14% of patients surviving more than 4 years, indicating the drug's potential benefits in real-world settings with complex patient profiles.

Background: About 20% of patients with renal cell carcinoma present with non-clear cell histology (nccRCC), encompassing various histological types. While surgery remains pivotal for localized-stage nccRCC, the role of cytoreductive nephrectomy (CN) in metastatic nccRCC is contentious. Limited data exist on the role of CN in metastatic nccRCC under current standard of care.

Background: Immune checkpoint inhibitors (ICIs) improve overall survival (OS) in advanced/metastatic urothelial cancer (a/mUC) patients. Preliminary evidence suggests a prognostic role of inflammatory biomarkers in this setting. We aimed to develop a disease-specific prognostic inflammatory index for a/mUC patients on ICIs.

Article Synopsis
  • - The ARON-2 study investigated the real-world effectiveness of pembrolizumab, an immune checkpoint inhibitor, in patients with advanced urothelial carcinoma who had progression after platinum-based chemotherapy, utilizing data from 836 patients across 88 institutions in 23 countries.
  • - Results showed median overall survival (OS) of 10.5 months and overall response rate (ORR) of 31%; those who progressed after initial chemotherapy (cohort A) had lower OS (9.1 months) compared to those who recurred within a year post-chemotherapy (cohort B) with 14.6 months OS.
  • - Multivariate analysis identified several prognostic factors affecting OS and progression-free survival (PFS),

The ARON-2 study (NCT05290038) aimed to assess the real-world efficacy of pembrolizumab in patients recurred or progressed after platinum-based chemotherapy. This retrospective analysis reports the outcomes of urothelial carcinoma (UC) patients with bone metastases (BM). Medical records of patients with documented metastatic UC treated by pembrolizumab as second-line therapy were reviewed from60 institutions in 20 countries.

The introduction of first-line combinations had improved the outcomes for metastatic renal cell carcinoma (mRCC) compared to sunitinib. However, some patients either have inherent resistance or develop resistance as a result of the treatment. Depending on the kind of therapy employed, many factors underlie resistance to systemic therapy.

Registrative trials recommended the use of upfront chemotherapy in high-volume metastatic prostate cancer. We reported survival outcomes of patients with high-volume mCRPC treated with ARTA in a chemo-naïve setting compared to patients treated with chemotherapy as first-line from a longitudinal real-life multicenter series. We retrospectively collected data on mCRPC patients treated at six centers.

In cancer, a patient is considered a survivor from the time of initial diagnosis until the end of life. With improvements in early diagnosis and treatment, the number of cancer survivors (CS) has grown considerably and includes: (1) Patients cured and free from cancer who may be at risk of late-onset cancer therapy-related cardiovascular toxicity (CTR-CVT); (2) Patients with long-term control of not-curable cancers in whom CTR-CVT may need to be addressed. This paper highlights the importance of the cancer care continuum, of a patient-centered approach and of a prevention-oriented policy.

Background: The advent of immune-checkpoint inhibitors has challenged previous treatment paradigms for advanced urothelial carcinoma (UC) in the post-platinum setting as well as in the first-line setting for cisplatin-ineligible patients. In this study, we investigated the effectiveness of pembrolizumab as first-line treatment for cisplatin-ineligible UC.

Methods: Data from patients aged ≥ 18 years with cisplatin-ineligible UC and receiving first-line pembrolizumab from January 1st 2017 to September 1st 2022 were collected.
