Article Synopsis
  • Pelvic gynecological surgeries often lead to complications due to damage to the autonomic nervous system, and recognizing the deep uterine vein (DUV) can help prevent this.
  • A systematic literature review and anatomical studies were conducted to better understand the DUV's structure and role in surgical settings, utilizing cadaver dissections and advanced imaging techniques.
  • The research confirmed the DUV's importance in dividing uterine regions and assisting surgeons in navigating the subperitoneal anatomy to reduce the risk of nerve and ureter injuries during procedures.

Background: Given the scarce donor supply, an increasing number of so-called marginal or extended criteria donor (ECD) organs are used for liver transplantation. These ECD liver grafts are however known to be associated with a higher rate of early allograft dysfunction and primary non-function because of a greater vulnerability to ischemia-reperfusion injury. The end-ischemic hypothermic oxygenated machine perfusion (HOPE) technique may improve outcomes of liver transplantation with ECD grafts by decreasing reperfusion injury.

Background: It is unclear whether preoperative biliary drainage (PBD) by endoscopic retrograde cholangiopancreatography (ERCP) is equivalent to electrocautery-enhanced lumen-apposing metal stent (ECE-LAMS) before pancreatoduodenectomy (PD).

Methods: Patients who underwent PBD for distal malignant biliary obstruction (DMBO) followed by PD were retrospectively included in nine expert centers between 2015 and 2022. ERCP or endoscopic ultrasound-guided choledochoduodenostomy with ECE-LAMS were performed.

Background: Factors excluding postoperative pancreatic fistula (POPF), facilitating early drain removal and hospital discharge represent a novel approach in patients undergoing enhanced recovery after pancreatic surgery. This study aimed to establish the relevance of neutrophil-to-lymphocyte ratio (NLR) in excluding POPF after pancreatoduodenectomy (PD).

Methods: A prospectively maintained database of patients who underwent PD at two high-volume centres was used.

Article Synopsis
  • - The study investigates the need for therapeutic drug monitoring of tacrolimus (TAC) in organ transplant patients due to its narrow therapeutic index and variability in blood concentrations.
  • - The research focuses on the impact of genetic variants and the expression of four drug transporters on TAC concentrations, finding that some transporters correlate with TAC levels in liver and kidney transplant recipients.
  • - In vitro experiments confirmed TAC as a substrate of the drug transporter P-glycoprotein (P-gp), while results for other transporters were inconclusive, enhancing our understanding of TAC transport mechanisms in T lymphocytes.

Article Synopsis
  • Researchers wanted to find out how to tell if a redo liver transplant (redo-LT) is successful or not, especially since there aren't many organs available to transplant.
  • They looked at data from 22 hospitals from 2010 to 2018, focusing on certain patients who had a better chance of having a good outcome after the surgery.
  • They found that while most patients had a high survival rate after 1 year, those who had redo-LT for specific complications did not do as well, showing that some cases are riskier than others.
