Background Context: Cage subsidence is a complication of interbody fusion associated with poor clinical outcomes. 3D-printed titanium interbody cages allow for the alteration of features such as stiffness and porosity. However, the influence of these features on subsidence and their biological effects on fusion have not been rigorously evaluated.

Background Context: 3D-printed titanium cage designs can incorporate complex, porous features for bone ingrowth and a greater surface area for minimizing subsidence. In a companion study (Part 1), we determined that increased surface area leads to decreased subsidence; however, it remains unclear how increasing the cage surface area, resulting in a smaller graft aperture, influences fusion.

Purpose: We evaluated the effects of surface area of 3D-printed titanium cages and the use of autologous bone grafts on spinal fusion in sheep.

Article Synopsis
  • Neurogenic claudication due to lumbar stenosis is common, and the mild (minimally invasive lumbar decompression) procedure is emerging as a safe alternative to traditional treatments, though complications are infrequently documented.
  • A case is presented where a patient experienced a cerebrospinal fluid leak after the mild procedure, leading to unsuccessful lumbar drainage and the need for two additional surgeries.
  • This highlights that even minor procedures can lead to serious complications, necessitating education for all healthcare providers involved on the associated risks and optimal management strategies.

Article Synopsis
  • - The study examines whether specialized treatment, specifically transference focused psychotherapy in a day hospital setting, can improve school attendance in adolescents with borderline and other personality disorders (PD).
  • - Of the 175 juvenile patients studied, 60% were absent from school before treatment, but results showed significant improvements in school attendance post-treatment, highlighting key psychological predictors influencing these changes.
  • - With the upcoming ICD-11 emphasizing functional impairment, the findings suggest earlier assessment and treatment could better address school dysfunction in adolescents with PD, particularly those with borderline and avoidant traits, while future research should explore optimizing treatment for these groups.

Lipoproteins are essential for lipid transport in all bilaterians. A single Apolipoprotein B (ApoB) molecule is the inseparable structural scaffold of each ApoB-containing lipoprotein (B-lps), which are responsible for transporting lipids to peripheral tissues. The cellular mechanisms that regulate ApoB and B-lp production, secretion, transport, and degradation remain to be fully defined.

In zebrafish, maternally deposited yolk is the source of nutrients for embryogenesis prior to digestive system maturation. Yolk nutrients are processed and secreted to the growing organism by an extra-embryonic tissue, the yolk syncytial layer (YSL). The export of lipids from the YSL occurs through the production of triacylglycerol-rich lipoproteins.

Objective: Depression and anxiety are associated with poor outcomes following spine surgery. However, the influence of these conditions on achieving a minimal clinically important difference (MCID) following lumbar spine surgery, as well as the potential compounding effects of comorbid depression and anxiety, is not well understood. This study explores the impact of comorbid depression and anxiety on long-term clinical outcomes following surgical treatment for degenerative lumbar spondylolisthesis.

Article Synopsis
  • Zebrafish are valuable for researching lipid metabolism and related human disorders, but a standardized high-cholesterol diet (HCD) protocol for them was lacking.
  • The study developed and tested a new HCD protocol, which led to notable hypercholesterolemia in the fish, indicated by increased levels of ApoB-containing lipoproteins and plasma cholesterol.
  • Results showed that while larvae developed stable hepatic steatosis after being fed the HCD, adult zebrafish did not maintain the fatty liver phenotype but did exhibit elevated levels of ApoB-LPs and lipogenesis gene activity.

Article Synopsis
  • The study investigates the effects of posterior column osteotomies (PCOs) combined with anterior lumbar interbody fusion (ALIF) on spinal curvature, specifically measuring changes in lordosis, in patients.
  • A total of 99 patients were analyzed, with results showing that those who underwent PCOs experienced significantly greater improvements in lumbar lordosis, segmental lordosis, and disc angle compared to those who had just ALIF.
  • The findings suggest that adding PCOs during ALIF procedures enhances spinal correction, allowing the disc angle to closely match the designed curvature of the interbody cage.

Objective: The mini-open lateral retropleural (MO-LRP) approach is an effective option for surgically treating thoracic disc herniations, but the approach raises concerns for pneumothorax (PTX). However, chest tube placement causes insertion site tenderness, necessitates consultation services, increases radiation exposure (requires multiple radiographs), delays the progression of care, and increases narcotic requirements. This study examined the incidence of radiographic and clinically significant PTX and hemothorax (HTX) after the MO-LRP approach, without the placement of a prophylactic chest tube, for thoracic disc herniation.

Objective: Recombinant human bone morphogenetic protein-2 (rhBMP-2) has been demonstrated to achieve the highest rates of arthrodesis in multilevel lumbar fusion but is also associated with possible perioperative morbidity. A novel allograft (OSTEOAMP) is a differentiated allograft that retains growth factors supporting bone healing. The authors sought to compare the clinical and radiographic outcomes of rhBMP-2 and the novel allograft in lumbar interbody arthrodesis to determine if the latter may be a safer and equally effective alternative to rhBMP-2 for single- and multilevel posterior or transforaminal lumbar interbody fusion (PLIF or TLIF).

To design new CARs targeting hepatitis B virus (HBV), we isolated human monoclonal antibodies recognizing the HBV envelope proteins from single B cells of a patient with a resolved infection. HBV-specific memory B cells were isolated by incubating peripheral blood mononuclear cells with biotinylated hepatitis B surface antigen (HBsAg), followed by single-cell flow cytometry-based sorting of live, CD19 IgG HBsAg cells. Amplification and sequencing of immunoglobulin genes from single memory B cells identified variable heavy and light chain sequences.
