A case of colopleural fistula, resulting from strangulation and perforation of a diaphragmatic hernia and presenting as tension pneumothorax, is reported. The hernia was most likely a consequence of a stab wound to the left side of the chest four years before admittance. Colopleural fistula as a cause of tension pneumothorax is an extremely rare entity, reported only once in past English medical literature.

Examination showed an increased incidence of right-sided colon cancer by decade and a simultaneous fall in the incidence of rectosigmoid lesions. A trend toward a more favorable Dukes' pattern was evident with each rise in decade.

The attrition of normal defences followed by inadequate reparative response leads to an increasing number of significant ailments while simultaneously muting their clinical manifestations. Greater recognition of the nature of these ailments and their protean presentation should improve their detection and, hopefully, the outcome for the patient.
