The surgical correction of short face syndrome was greatly improved with the use of miniplate fixation which reduces dramatically the rate of vertical relapse. This work reports on five adult patients treated with an unusual Le Fort I osteotomy because no pterygomaxillary separation was done. The Le Fort I osteotomy was isolated (1 case) or associated with a mandibular advancement (3 cases) or a vertical augmentation genioplasty (1 case).

A dento-alveolar origin seems to be more frequent than a squelettic one; in our experience, there is a direct relationship between the volume of the tongue and the open bite--surgical correction was done with a LE FORT 1 osteotomy and a tongue reduction was done simultaneously in half of the patients--a sliding genioplasty sometimes was done to improve the profile--for us, post chirurgical relapse was frequent with patients without tongue reduction.

The chin position in overall facial aesthetics is well known and it has an important influence on personality and relations with others. For most plastic surgeons, genioplasty is limited to chin advancement with alloplastic implants. Sliding horizontal osteotomy of the mandibular symphysis is not well known: the technique is simple, fast and safe.
