A 79 year old white women presented with a severe bleeding disorder. Evaluation revealed a prothrombin time of 27.6 seconds (control, 11 seconds) and an activated partial thromboplastin time of 61 seconds.

He developed mild aortic insufficiency, A-V block and pericardial effusion. Painful ophthalmoplegia occurred 5 months later with exacerbation of the cardiac complications as corticosteroids were being tapered.

Five had required surgical treatment for relief of obstructive symptoms or because of diagnostic uncertainty. Immunological studies, including serum immunoglobulins, serum complement, lymphocyte quantitation, and lymphocyte response to nonspecific mitogens revealed no striking abnormalities.
