Background And Purpose: The dorsolateral portion of the caudal pons contains the vestibular nucleus (VN) and inferior cerebellar peduncle (ICP) that play important roles in conveying and processing vestibular and ocular motor signals. This study aimed to characterize ocular motor abnormalities along with their anatomical correlations in dorsolateral pons (DLP) lesions.

Methods: We analyzed clinical features, and results of neuro-otological evaluations and neuroimaging of 18 patients with unilateral DLP lesions (17 with DLP infarction and 1 with cavernous malformation) from among 506 patients with pontine infarction in a stroke registry.

Background: Many individuals use the Internet, including generative artificial intelligence like ChatGPT, for sleep-related information before consulting medical professionals. This study compared responses from sleep disorder specialists and ChatGPT to common sleep queries, with experts and laypersons evaluating the responses' accuracy and clarity.

Methods: We assessed responses from sleep medicine specialists and ChatGPT-4 to 140 sleep-related questions from the Korean Sleep Research Society's website.

Saccadic oscillations (SOs) mostly occur spontaneously, but can be occasionally triggered by various stimuli. To determine clinical characteristics and underlying mechanisms of triggered SOs, we analyzed the clinical features and quantitative eye-movement recordings of six new patients and 10 patients in the literature who exhibited with triggered SOs. Eleven of the 16 patients (69%) had a lesion involving cerebellum and/or brainstem such as cerebellar degeneration, cerebellitis, or cerebellar infarction.

Cronkhite-Canada syndrome is a rare gastrointestinal polyposis syndrome with distinctive clinical features and endoscopic findings. Diagnosis can be challenging without suspicion, and the disease carries high mortality due to complications such as infection, gastrointestinal bleeding, and malignancies. This paper presents two cases of Cronkhite-Canada syndrome occurring after coronavirus disease 2019 (COVID-19) mRNA vaccination.

Background And Purpose: The neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) have been identified as useful biomarkers for assessing the inflammatory response and for predicting the prognosis of various diseases. This study aimed to determine the clinical significance and effects on prognostic prediction of NLR and PLR in acute unilateral vestibulopathy (AUV).

Methods: We retrospectively recruited 128 patients who were diagnosed with AUV from July 2016 to April 2021, and compared NLR and PLR values between these patients with AUV and age- and sex-matched healthy subjects.

Background/aims: We investigated the incidences of overall and site-specific malignancies and chemopreventive effects of statin, metformin, and aspirin in patients with ulcerative colitis.

Methods: We collected data using the Health Insurance Review and Assessment claims database from January 2007 to April 2020.

Results: The overall malignancy risk among the 35,189 ulcerative colitis patients was similar to that of the general population (standardized incidence ratio, 0.

Article Synopsis
  • - This study explored the causes of recurrent benign paroxysmal positional vertigo (BPPV) in young individuals by analyzing gene expression and using bioinformatics techniques.
  • - Researchers extracted RNA from blood samples of four young BPPV patients and four controls, identifying 39 differentially expressed genes (with more being upregulated) that are linked to metabolic and immune system processes.
  • - The study concluded that oxidative stress and inflammation-related extracellular matrix degradation might play roles in developing recurrent BPPV among young patients.

Background/aims: This study aimed to evaluate the characteristics of endoscopic retrograde cholangiopancreatography (ERCP) or endoscopic ultrasonography (EUS)-related adverse events (AEs) that eventually lead to medical disputes or claims on medical professional liability.

Methods: Medical disputes for ERCP/EUS-related AEs filed in the Korea Medical Dispute Mediation and Arbitration Agency between April 2012 and August 2020 were evaluated using corresponding medical records. AEs were categorized into three sections: procedure-related, sedation-related, and safety-related AEs.

Colonoscopic polypectomy is effective in decreasing the incidence and mortality of colorectal cancer (CRC). Premalignant polyps discovered during colonoscopy are associated with the risk of metachronous advanced neoplasia. Postpolypectomy surveillance is the most important method for managing advanced metachronous neoplasia.

  • Device-assisted enteroscopy (DAE) is a new method that helps doctors see and treat problems in the small intestine better than before.
  • Unlike capsule endoscopy, DAE allows doctors to closely look at the area and take tissue samples for testing.
  • The Korean Association for the Study of Intestinal Diseases created a guide to help doctors know when and how to use DAE safely and effectively.

The introduction of device-assisted enteroscopy (DAE) in the beginning of the 21st century has revolutionized the diagnosis and treatment of diseases of the small intestine. In contrast to capsule endoscopy, the other main diagnostic modality of small bowel diseases, DAE has the unique advantages of allowing the observation of the region of interest in detail and enabling tissue acquisition and therapeutic intervention. As DAE becomes an essential procedure in daily clinical practice, there is an increasing need for correct guidelines on when and how it is to be performed and what technical factors should be taken into consideration.

The incidence and characteristics of acute vestibular symptoms, responsible structures, and lateralization of the causative lesions in supratentorial stroke remain unknown. This study aimed to determine the incidence, clinical features, and anatomical correlation of acute vestibular symptoms in supratentorial stroke. We performed a prospective, multicenter, observational study that had recruited patients with supratentorial stroke from the neurology clinics of referral-based four university hospitals in Korea.

Article Synopsis
  • - This study investigates a specific form of X-linked adrenoleukodystrophy (X-ALD) characterized by symptoms affecting the cerebellum and brainstem in a family with three affected members.
  • - A thorough evaluation identified similar symptoms like slurred speech and ataxia, but the severity and imaging results varied among the affected individuals; elevated very long-chain fatty acids (VLCFA) were found in all, while only one had adrenal dysfunction.
  • - Genetic analysis revealed a mutation (c.887A>G, p.Tyr296Cys) in the ATP-binding cassette subfamily D member 1 gene, suggesting it disrupts protein structure, demonstrating the diverse clinical presentation of X-ALD

Article Synopsis
  • Vertical pendular nystagmus (PN) is a rare eye movement phenomenon that can occur in patients with acute pontine lesions, typically accompanied by significant neurological deficits like locked-in syndrome.
  • An analysis of a new case alongside 11 documented cases revealed commonalities, such as horizontal gaze palsy and variability in vertical oscillation frequency and amplitude.
  • The study suggests two potential mechanisms for acute vertical PN, either due to disruption of the central tegmental tract impacting the inferior olives or damage to omnipause neurons resulting in low-velocity saccadic oscillations.
