We report the case of a patient with an incidental diagnosis of gallbladder adenocarcinoma with chondrosarcomatoid areas in a cholecystectomy specimen. Since it is associated with a worse prognosis when compared to usual carcinoma, we need to understand this entity to offer our patients a better treatment.

Surgery is scheduled for removal of the tumor, during which it is observed that the lesion originates in the ileum. The histopathological study shows a neoplasm of small round cells with nucleoli and scant cytoplasm.

Methods: We report a new case of MCN and describe the radiological characteristics of this neoplasia on intravenous urography (IVU), pyelography , CT scan and MRI.
Results: CT scan and MRI findings are highly suggestive of multilocular cystic nephroma.
