[The MDK as an Institution for Quality Assurance in the German Health System: A Practice Report on Quality Audits Based on the Guideline on Minimally Invasive Heart Valve Interventions of the G-BA].

Introduction: In 2015, the Federal Joint Committee (G-BA) adopted a guideline on minimally invasive heart valve interventions (MHI-RL). We present an evolutionary analysis of the relative time of genetic events underlying tumorigenesis in human bladder cancers from 10 whole cystectomy specimens using multiregional whole-exome sequencing. We timed bladder cancer drivers, mutational signatures, ploidy and copy number alterations, provided evidence for kataegis and correlated alterations with tumour areas and histological phenotypes. We found that: (1) heterogeneous tumour areas/phenotypes had distinct driver mutations, (2) papillary-invasive tumours divided early into two parallel evolving branches and (3) parallel evolution of subclonal driver mutations occurred.

Parameter dependence of high-frequency nonlinear oscillations and intrinsic chaos in short GaAs/(Al, Ga)As superlattices.

We explore the design parameter space of short (5-25 period), n-doped, Ga/(Al,Ga)As semiconductor superlattices (SSLs) in the sequential resonant tunneling regime. We consider SSLs at cool (77 K) and warm (295 K) temperatures, simulating the electronic response to variations in (a) the number of SSL periods, (b) the contact conductivity, and (c) the strength of disorder (aperiodicities). Our analysis shows that the chaotic dynamical phases exist on a number of sub-manifolds of codimension zero within the design parameter space.

Structural and compositional characterization of ultrathin titanium oxide films grown on Pt3Ti(111).

We have investigated the growth of ultrathin titanium oxide (TiO(x)) films on a Pt(3)Ti(111) single crystal surface as a function of oxidation temperature (300-1000 K) and oxygen exposure (up to 4500 l) by means of Auger electron spectroscopy, low-energy electron diffraction, ultraviolet photoelectron spectroscopy and high-resolution electron energy loss spectroscopy (HREELS). Both the surface composition and the surface structure of the resulting TiO(x) films exhibit a strong dependence on the preparation conditions. Loss of the chemical order and Ti segregation are observed at the Pt(3)Ti(111) surface upon oxygen exposures of more than 135 l at 1000 K.

Due to surgery duration variability and arrivals of emergency surgeries, the planned Operating Room (OR) schedule is disrupted throughout the day which may lead to a change in the start time of the elective surgeries. These changes may result in undesirable situations for patients, wards or other involved departments, and therefore, the OR schedule has to be adjusted. In this paper, we develop a decision support system (DSS) which assists the OR manager in this decision by providing the three best adjusted OR schedules.

[Metastatic apocrine adenocarcinoma of the penis: cytostatic therapy].

Metastatic carcinoma of the genital apocrine sweat glands is a rare clinical entity. A cytostatic treatment with MTX and bleomycin was performed empirically but did not show any satisfactory efficacy. The conversion of the chemotherapeutic regime to a tyrosine kinase inhibitor (sunitinib) showed a morphologically visible and clinically proven success in terms of a significant slowing down of the previous rapid progression of the disease.

Repair of massive rotator cuff tears in the elderly can be challenging because of degenerative, retracted, and contracted tendons producing an extensive defect. The quality of the tendon and particularly the bone of the proximal humerus is an inherent weakness in the repaired construct. We report a method that secures the rotator cuff to a fixation post placed in cortical bone at the surgical neck of the humerus, bypassing the weakest point.
