Severe hypertriglyceridemia in pregnancy, defined as triglycerides >1000 mg/dL, is a rare but high-acuity condition that can precipitate several complications for the mother and fetus, in particular hypertriglyceridemia-induced acute pancreatitis (HTGAP). The treatments employed in the management of hypertriglyceridemia-induced acute pancreatitis (HTGAP) have the potential to significantly alter the anesthetic course of an impending or emergent delivery. In this report, we present two cases of HTGAP, one complicated by a concomitant COVID-19 infection and each with a unique approach to anesthetic management in the setting of two very different and cofounded clinical presentations.

Congenital myopathies raise unique challenges for anesthesiologists during labor and delivery. Apart from having a risk for malignant hyperthermia, this patient population can present with severe restrictive lung disease in the third trimester. Scoliosis and weak pelvic muscles could make regional anesthesia difficult.

Introduction: The Bronx is a borough of New York City that has been profoundly affected by the COVID-19 pandemic. Limited reports exist discussing the anaesthetic management of obstetric patients infected with COVID-19. We review a cohort of obstetric patients in the Bronx with COVID-19 and report their delivery data, anaesthetic management, and maternal-fetal outcomes.

Introduction: Opioid exposure during hospitalization for cesarean delivery increases the risk of new persistent opioid use. We studied the effectiveness of stepwise multimodal opioid-sparing analgesia in reducing oxycodone use during cesarean delivery hospitalization and prescriptions at discharge.

Methods: This retrospective cohort study analyzed electronic health records of consecutive cesarean delivery cases in four academic hospitals in a large metropolitan area, before and after implementation of a stepwise multimodal opioid-sparing analgesic computerized order set coupled with provider education.

Background: The rate of cesarean delivery is on the rise in the USA. Satisfactory postoperative analgesia remains a top priority for cesarean delivery. Multimodal analgesia with a reduction in opioid consumption and improvement in patient satisfaction is a goal for anesthesiologists caring for this patient population.
