Article Synopsis
  • A study assessed a new ultrasound-based algorithm for determining when to remove pancreatic stents placed to prevent post-ERCP pancreatitis, aiming to minimize unnecessary X-ray imaging.
  • Out of 88 patients, the ultrasound approach successfully reduced the need for X-rays by 74%, accurately visualizing stents in 81% of cases but with limited specificity.
  • The authors recommend implementing the ultrasound algorithm with training for experienced personnel due to challenges with visualizing shorter stents.

Background: Given the geographical sparsity of Rare Diseases (RDs), assembling a cohort is often a challenging task. Common data models (CDM) can harmonize disparate sources of data that can be the basis of decision support systems and artificial intelligence-based studies, leading to new insights in the field. This work is sought to support the design of large-scale multi-center studies for rare diseases.

Background: Predictors of poor outcome associated with variceal bleeding remain suboptimal. In patients with cirrhosis, serum lactate combined with Model for End-Stage Liver Disease (MELD-LA) improved prediction across heterogeneous populations. However, prognostic properties have not yet been assessed in the context of variceal bleeding.

Article Synopsis
  • The study emphasizes the importance of accurately assessing risks in patients with Acute-on-Chronic Liver Failure (ACLF), as it is linked to high short-term mortality, particularly for liver transplant decisions.
  • The research found that patients with pulmonary impairment face significantly higher mortality rates, with those experiencing pulmonary failure having an alarming 83.7% mortality within 28 days.
  • Adjusting the CLIF-C ACLF score to account for the level of pulmonary impairment can improve the prediction of outcomes, suggesting that both pulmonary failure and mechanical ventilation are critical factors for evaluating the prognosis of ACLF patients.

Background: Gastrointestinal bleedings (GIBs) are frequent in cirrhotic patients and lead to high morbidity and mortality. Lately, there have been conflicting reports on the role of and bleeding type [variceal bleeding and nonvariceal bleeding (NVB)]. This study investigated the predictors of mortality in patients with variceal bleeding and NVB with relationship to sex differences.

Background & Aims: The relationship between acute-on-chronic liver failure (ACLF) and acute variceal bleeding (AVB) is poorly understood. Specifically, the prevalence and prognosis of ACLF in the context of AVB is unclear, while the role of transjugular intrahepatic portosystemic shunt (TIPS) in the management in patients with ACLF has not been described to date.

Methods: A multicenter, international, observational study was conducted in 2,138 patients from 34 centers between 2011 and 2015.

Human and animal African trypanosomiasis (HAT & AAT, respectively) remain a significant health and economic issue across much of sub-Saharan Africa. Effective control of AAT and potential eradication of HAT requires affordable, sensitive and specific diagnostic tests that can be used in the field. Small RNAs in the blood or serum are attractive disease biomarkers due to their stability, accessibility and available technologies for detection.
