An adult patient was admitted to our emergency department for a first episode of generalized tonic-clonic seizure. Computed tomography scan and magnetic resonance imaging showed a temporal intracranial hemorrhage and parenchymal edema caused by a dural arteriovenous fistula (DAVF), whose angioarchitecture was better understood through the DSA which showed as intriguing and rare vascular anomaly the origin of the posterior inferior cerebellar artery (PICA) from the middle meningeal artery (MMA). The endovascular treatment of the DAVF was then successfully performed.

Coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), typically affects the respiratory system but can also present with neurological manifestations. Although some cases of hydrocephalus related to COVID-19 infection have been reported, a clear association between these two entities is not universally recognized yet. A 75-year-old man presented with bilateral frontal hematomas owing to a dural arteriovenous fistula involving an isolated segment of the superior sagittal sinus. Most medical devices are routinely recognized on radiological images and described as normal findings in the radiological report, but sometimes they can cause patient access to the emergency department. Multiple possible complications have been described and most of them require prompt recognition by radiologists for proper clinical management. This commentary proposes a systematic approach to radiological reporting of the most common emergent complications related to medical devices with the intent to avoid the omission of important findings in the final radiological report.

Methods: We retrospectively enrolled 25 patients with AIS due to LVO and with a CTP study showing the presence of ischemic penumbra who underwent effective MT, regardless of the time of onset. The controls were 25 AIS patients with overlapping demographics and clinical and computed tomography angiography features at admission who had undergone successful MT within 6 h from onset and without a previous CTP study.
