[The influence of induced severe myocardial ischemia on coronary disease course].

The aim is to study influence of short-term episodes ischemic of myocardium in a combination with intravenous introduction of creatine phosphate on current of ischemic heart disease at the patients with distal stenoses of coronary arteries. We survey 86 patients, which were divided into two groups: basic, which are receiving neoton in a combination with ischemic training and control who are receiving standard antianginal therapy. The research proceeded within 5 years.

[Sacral anesthesia in surgical treatment of soft tissues neoplasms of the lower extremities].

The experience with sacral anesthesia in surgical treatment of soft tissues' benign and malignant neoplasms of the lower extremities was analyzed. Benign tumors were the indications for surgical treatment in 19 patients, malignant tumors - in 11 patients. Various methods of sacral anesthesia are described, indications for this type of anesthesia and obtained results are discussed.

[Role of some wild birds and mammals in the natural foci of Crimean haemorrhagic fever in Stavropol' region].

The results of the study on the role of some wild mammals and birds as feeding sources of for ticks Hyalomma marginatum, the main vectors of Crimean haemorrhagic fever virus in the Stavropol Territory, in the preimago phases of their development are presented. These phases (larvae and nymphs) were found on rooks, hooded crows, partridges, European brown hares and eared hedgehogs. The examination of domestic fowl resulted in finding larvae and nymphs in small amounts on turkeys.

[Modern endosurgical technologies in the diagnosis and treatment of maxillary inflammations and tumors (part 2)].

A total of 102 patients with inflammatory diseases and tumors of the maxillary sinuses were examined and treated using endosurgical technologies. X-Ray computer-aided tomography and magnetic imaging helped determine the access to the maxillary sinus, and the endosurgical instrument was inserted through the anterior wall of the sinus, medial wall, and maxillary tuberosity. Endosurgery was the most effective in polypous sinusitis, mucocele, inverted papilloma, and for biopsy.

[Method of laparoscopic treatment of gastroesophageal reflux with mesh implant].

Modern potential of endoscopic surgery promoted expansion of indications for surgical treatment of gastroesophageal reflux disease. The majority of laparoscopic methods has a number of significant disadvantages. Methods of laparoscopic treatment of gastroesophageal reflux using the implant with partial anterior fundoplication or without it were developed.

[Endoscopic removal of the gallbladder mucosa after cholecystostomy].

The authors have developed a new method of radical treatment of patients with acute obstructive cholecystitis after cholecystostomy. The endoscopic removal of the mucosa was proposed using hysteroresectoscope GYI-525-12 (Circon-ACM1). The method was used in 5 elderly patients with high anesthesia risk in whom surgical cholecystectomy was not possible.

[Laparoscopic treatment of hydatid liver cysts].

Laparoscopic hepatic hydatidectomy (LHE) was performed in 37 patients. Conversion to laparotomy in one case was caused by intrahepatic location of liver cyst. In 36 patients after LHE, the postoperative period was significantly easier than after standard operation.

Results of treatment of 162 patients with echinococcosis of the liver were analyzed. The present status of gastroenterological laparoscopic surgery in Russia is comprehensively analyzed. Based on the generalized data obtained by 11 leading clinicians of the country, the authors in detail consider the feasibilities of endoscopic techniques of surgical gastroenterology, the proportion of each nosological entity in the total statistics of laparoscopic operations. A description is given of the following endoscopic interventions: cholecystectomy, choledocholithotomy, biliodigestive anastomoses, vagotomy, perforated ulcer suture, antireflux operations, gastrectomy, sigmoidectomy, right-sided hemicolectomy, rectal surgery, appendectomy, Meckel's diverticulum removal.

[Possibilities of the use of trypsinogen activation peptide in the pancreatic tissue in the diagnosis of pancreatic necrosis].

The test of peptide of trypsinogen activation (PTA) in various biological media is a principal new diagnostic test for acute pancreatitis. PTA levels in the pancreatic tissue and the plasma of the blood in acute experimental pancreatic necrosis reflects the process of the pathological activation of trypsin in the diseases focus and confirms the source of higher levels of activation peptide in the blood flow. Together with the values of other enzymatic systems of the pancreatic gland the discussed value permits one to make a more precise assessment of the mechanism and stages of autodigestive processes which develop in acute experimental pancreatitis.

[The effect of exogenous protein--a proteinase inhibitor--on the development of experimental pancreatitis in dogs].

Small doses of an exogenous protein inhibitor of proteinases ovomucoid, isolated from duck egg-white, exhibited distinct therapeutic effects in acute pancreatitis of dogs. The inhibitor decreased the lethality rate and exceeded the base Kunitz trypsin inhibitor in its efficiency.

[The morphogenesis of pancreonecrosis].

Pancreas was studied histologically, histochemically and electron microscopically in 117 autopsy cases and in the experiment (25 dogs and 60 white rats). Morphogenesis of pancreonecrosis proceeds through certain stages. The initial stages of an acute pancreatic edema is characterized by degenerative and necrotic changes of pancreocytes with parapedesis of pancreatic enzymes into the interstitium and a reactive stromal edema.

[Evaluation of radical lymphadenectomy in breast cancer].

The study was concerned with establishing parallel clinico-roentgeno-morphological features in diagnosis of lesions in axillary lymph nodes in 128 breast cancer patients. Complex evaluation of lymphographic and palpation evidence proved valuable because its application allows to avoid diagnostic errors for a certain stage of the disease. Also the results of X-ray postoperative evaluation of radicality of lymphadenectomies carried out in 100 mastectomies after Halsted, 22 mastectomies after Patey and 15 axillary lymphadenectomies are presented.
