Percutaneous mitral valve repair with the MitraClip system recently emerged as a viable and less invasive therapeutic option in patients with severe mitral regurgitation deemed to be high-risk surgical candidates. Mitral valve morphology and geometry features are key elements for MitraClip eligibility. In the setting of functional mitral regurgitation, the presence of a leaflet coaptation gap due to advanced left ventricle remodeling can be a potential exclusion criterion for MitraClip therapy.

Article Synopsis
  • * Researchers analyzed data from 151 patients, finding that excessive oversizing significantly predicted higher postoperative gradients, with an odds ratio indicating a clear risk factor.
  • * The findings suggest that avoiding excessive oversizing is crucial and highlight the need for improved sizing strategies, possibly using cardiac CT, to ensure better outcomes for patients receiving the Perceval valve.

Article Synopsis
  • Transcatheter aortic valve implantation (TAVI) is a procedure used to treat severe aortic valve stenosis, particularly in patients who are at high risk for traditional surgery.
  • An 80-year-old woman with severe stenosis and mild surgical risk opted for TAVI instead of traditional surgery, leading to the implantation of a Lotus valve via the transfemoral route.
  • During the procedure, she suffered interscapular pain and was found to have a rare type B aortic dissection, which was successfully treated with a vascular endoprosthesis, highlighting the unpredictable nature of complications associated with TAVI.
