Background: Stubborn dyschromia such as melasma and post-inflammatory hyperpigmentation (PIH) are leading causes for cosmetic consultation. Topical treatment is challenging, using a range of modalities, to stop, hinder, and/or prevent steps in the pigment production process. Tranexamic acid (TXA), a potent plasmin inhibitor, is proposed to control pigmentation by inhibiting the release of inflammatory mediators involved in triggering melanogenesis.

We describe a case of melanoma with the presence of comedo-like openings at dermoscopy. These structures, typical of seborrheic keratosis, represent an uncommon finding in melanoma. We emphasize the importance of searching for specific dermoscopic criteria for melanocytic lesions during the examination of a pigmented lesion, despite possible observations of characteristic structures of non-melanocytic lesions, in order to increase the accuracy in the diagnosis of melanoma.

A 65-year-old man presented with an approximately 5-year history of cutaneous nodules of the arms and legs. In addition, physical examination revealed bilateral thickening of ulnar and tibial nerves, distal weakness with hallux extension and finger abduction, and distal hypoesthesia of the left hallux. Histopathological analysis of the skin biopsy specimen showed a dense inflammatory infiltrate in the hypodermis, characterized by vacuolated macrophages containing multiple organisms.
